When a complaint gets filed, the investigation stops being about what you remember and becomes entirely about what you can prove. Your technique is invisible to an investigator — your documentation is everything.
These are the 6 fields that decide whether your chart protects you or exposes you:
1. Product, lot # & expiration
2. Injection map + units per site
3. Consent — specific & dated
4. History + contraindication screen
5. Photos + a separate authorization
6. Adverse events + aftercare
Pull up your last five notes and check them against this list. If one of the six is consistently missing, that's the gap an investigator would find first.
