Terminal lucidity is one of the strangest things in neurology—people near death, even with severe brain injury or organ failure, suddenly snap into total mental clarity. The body is shutting down, but for a brief window, the brain lights back up.

Scientists think it might happen when inhibitory brain networks finally collapse, releasing a surge of activity, or when dopamine and glutamate flood the system as cells die. Oxygen fluctuations and metabolic shifts may also temporarily lift the fog of consciousness. In that window patients have been known to speak clearly, recognize loved ones, or express peace, right before the final decline. It's not mystical, it's neurobiology, but it still raises huge questions about where exactly the line between life and death really sits.
