My Ambulatory Care APPE at Christ Community Health Services West Tennessee – Broad Avenue provided valuable opportunities to strengthen my clinical reasoning, pharmacotherapy knowledge, and confidence in delivering evidence-based, patient-centered care.
Throughout this rotation, I had the opportunity to(but not limited to):
• Critically appraise and present the SYNCHRONIZE-2 clinical trial, evaluating survodutide for chronic weight management.
• Develop and present a comprehensive diabetes mellitus topic discussion, emphasizing guideline-directed management and individualized pharmacotherapy.
• Conduct and present 3–4 patient workups daily, focusing on T2DM, HTN, and warfarin management while developing evidence-based treatment recommendations.
• Perform medication reconciliations to assess medication safety, appropriateness, and adherence.
• Provide patient education on diabetes injectable therapies and inhaler techniques, promoting safe and effective medication use.
I would like to thank my preceptor, Dr. Nicole Northcutt, for the guidance and learning opportunities provided throughout this rotation.
7/10 APPE rotations completed! Each rotation continues to sharpen my clinical skills, strengthen my confidence, and bring me one step closer to becoming the pharmacist I aspire to be❤️💊🥼✨
