“Free at the point of use does not mean collapse at the point of need.”

On @gmb today I debated the idea of an A&E fee for clearly non urgent attendances.

My stance is simple.
Triage first. Charge after.
No paywall at the door. Everyone assessed. And if you come to Accident and Emergency for something that is not an accident, and not an emergency, then yes, there’s a fee. With exemptions for vulnerable groups. And every penny ring fenced into faster urgent care alternatives.

People don’t come to A&E to break the system. They come because they’re scared and I’m certainly not looking at a fee for punishment or ridicule. More for protection for an overcrowded service.

Protection from harm to the patient on a trolley in a corridor waiting for a bed. Harm to the person in the waiting room whose chest pain is missed in the noise. Harm to the exhausted nurse doing the work of three people. Harm to the doctor who leaves a night shift feeling like they failed a waiting room full of frightened humans.

The harm has to stop.

I’ve cared for an A&E doctor who reached crisis point after a shift. She went home and tried to take her life due to the pressure of relentless demand. That”s stayed with me and changed how I think about “no harm in just popping to A&E.”

I see A&E as the safety net. It should catch heart attacks, strokes, sepsis, major trauma. When it becomes the default front door when other front doors exist,- pharmacies, GP surgeries, walk in centres - there’s a price to pay. Literally.

Do you think a post triage fee is a fair nudge?
Wishing you a happy, healthy new year- and one where you don’t need to go to A&E!

A vital reminder that incredible change is always possible when you choose to take ownership of your health.

That’s the thought running through my mind as I watch and rewatch this video.
And it makes me wonder.
What’s the thought running through yours?

I rebuilt my life at 39.
At that time, it was not because I wanted to live longer, it was because I needed to live better.
Not to count time, but to create time.
Time to take care of myself, while taking care of others.

And now I want both.
I want a life that is both long and healthy.
A life where my lifespan matches my healthspan.

That’s what I call a Million Hour Life.
That’s 114 vibrant, meaningful years.

So let me ask you...why would a million hours of living longer, younger, stronger matter to you?

“Your brain makes thousands of decisions a day. Most of us have no idea how well it’s doing them.”

Longevity and lifestyle doctor @dralkapateluk joins us to talk about what keeps a brain healthy and why she scans with Connectome.

She’s data-driven to the core: using real measurements to help people make decisions about their health with more precision than guessing.

Her take on brain health comes down to four things. Feed it the right nutrients. Keep oxygen and fuel flowing to it. Protect your ability to focus. And give it time to float, which is where sleep comes in.

After years in practice, Alka has seen how data and the right information at the right time can make a real difference to how people feel and function.

That’s central to what we’re building at Connectome: brain health you can see and track over time, so what you do for it actually works.

Meet @dralkapateluk and hear what drew her to Connectome. by @dralkapateluk

We might be tracking the wrong organ.

We count steps, we count sleep, we watch our resting heart rate drop by two and feel pleased with ourselves. Meanwhile the organ you make every decision with is the one asset we’re not looking at enough

I’m on the educational stage at @pauselive today. 2pm on wearables and what your numbers are actually telling you. 3.30pm on brain health. Yes, your brain needs you! 🫵

Convene Sancroft, St Paul’s. Come to either, come to both. Say hello afterwards, I would love to meet you.

@grazia asked me what I do in the first half hour after I wake up. Three things, most days.

Light before laptop. Outside within 30 minutes if I can, because morning light helps set your body clock, and that clock has a say in how alert you feel now and how well you sleep tonight.

Move before sitting. Calf stretches while I brush my teeth. A wall squat before the shower.

Water before food. Even mild dehydration can blunt your focus.

Thanks @ambermiddletonlol for the question!

(ps this is not perfection every single morning 😁my children will tell you on some mornings they see me sitting in bed on my laptop in the dark! Rare, but still happens, because sometimes life demands it. But once you know what the better options are then you tend to lock in the longevity levers much more often)

What about you? What gets your morning started - on most days 😊?

Around 3 in 10 women invited for breast screening in England don’t go.

Please go. A mammogram can find a cancer long before you can feel it.

But breast screening is usually one size fits all. Same age, same frequency gap, for everyone.

Yet roughly 3 in every 1,000 women with a normal mammogram are diagnosed with cancer before their next screen is due. Dense breasts can make that more likely, partly because the density makes mammograms harder to read.

Last month I found a lump. A normal scan and mammogram, followed by intense relief, followed by a question. How often should I be screening now? My breasts are dense, grade C, so I’m adding an MRI to my screening portfolio. Yours could look completely different.

That’s personalised medicine.

Message me if you’d like to know what your personalised screening interval should be, based on your history, your lifestyle, your density, and I’ll send you a questionnaire.

💛 💛

What happens when you put a longevity doctor, a burnoutologist, a life coach, a nutritionist and a trailblazing TV director together?

A phenomenal night where nobody gets away with saying “I’m fine.”

Here’s the bit I most want you to hear. Burnout lives in your body too. The go go go, the broken sleep, the glucose swings can all feed inflammation, and inflammation can speed up how fast you age.

There’s no single test for burnout. But your biological age can show it coming. If your body is ageing faster than your years, burnout may already be on your heels, sometimes before you’ve noticed a thing.

If you’d like to know your biological age, message me and I’ll tell you about the test 💛

How much of your health is built around what you’re trying to avoid?

Fear can get you started. It’s wanting something that keeps you going. More energy. More strength. A clear head on a busy afternoon.

So what are you walking towards this week?

Send this to someone who could do with turning around today 💛

Have you ever listened to a recording of your own voice and thought, who is that?! I’ve done that too. 😁

But this week I discovered your voice can reveal more about you than you might think. Researchers listened to nearly 3,000 people talk, picking up their pace, pauses and pitch, and could estimate how old they were. They gave them a speech age. And when someone sounded older than their age, their brain often looked older on imaging too.

So interesting. Speech is your biology made audible.

It’s all in today’s Million Hour Memo. Plus three things you can do to lower your speech age and brain age. Message me if you’d like it in your inbox 💌

Have you ever done what I did? Woken up feeling unwell and still gone to work. We do that, don’t we? Push through. We think we’ve got enough reserve to just keep going, because we don’t want to let anyone down. Don’t want to look incapable.

The trouble with burnout is that it strips you of that reserve. I found that out the hard way. A simple fever I should have been able to shake. Instead it took hold and I ended up in hospital, seriously ill not knowing if I would see my children again.

That was fifteen years ago, and the gratitude for life itself has never left me. Neither has the whole new perspective it gave me on health and on what this body actually needs.

What does it need for us to realise we’re worth it?

That’s why I talk about burnout. I lived it. And I see so many women doing exactly what I did, striving hard, pushing on, never stopping.

So I’m really looking forward to Monday at @sohohouse with @primetimereset__ talking about exactly this. If you’ve ever gone to work when you should have been taking care of yourself, you’ll know what I mean.

Send this to the woman you know who never takes a day off.

The Fine Line. Someone told me today that their tests are fine and they feel fine. Which got me thinking.

You have tests. The results come back fine. Off you go, reassured. But how reassuring is that really?

Most of medicine is designed to look for what’s already gone wrong. Pathology. Longevity medicine is designed to look for what’s changing. Patterns. What I’m looking for is the direction you’re heading in. The drift, long before anything crosses a line.

And the lines we draw are strange when you actually look at them. An HbA1c of 47 and you don’t have diabetes. 48 and you do. A kidney filtration rate of 61 and you’re fine. Sitting at 59, and now it’s chronic kidney disease.

It’s a fine line, isn’t it. But it isn’t fine to give false reassurance. Doctors need to stop doing that.

Symptoms turn up late. A diagnosis turns up even later.

So the time to look more closely is when you feel fine. That’s when you can still choose which side of the fine line you stay on.

Health is definitely not the absence of disease!

When were you last reassured you were fine, when you didn’t feel it? Or felt completely fine, and only realised years later that you weren’t?
