🧐I have had many patients ask lately about reducing the risk of multiples in an ovarian stimulation-IUI cycle, and wanted to reshare a study of mine in @greenjrnl that addresses that topic!

🎉Honored this paper was chosen by @aboardobgyn as a Maintenence of Certification article for all board certified general OB/GYN and REI physicians!
⁣
➡️ 50,473 IUI cycles over 13 years.⁣
⁣
➡️ If < 40 yrs of age, more than 2 follicles on trigger day doesn't increase chances of success and may be dangerous. SINGLETON pregnancy rates stay essentially the same, while multiples risk increases 👶🏻👶🏻👶🏻. ⁣
⁣
🧐Sub-analyses: similar risk of multiples for PCOS & unexplained infertility.⁣
⁣
Check out/zoom in on the figures that help ⬇️ the risk of a pregnancy with multiples based on age and follicle number!

Via @drblakeevans: Excited to share my most recent publication that is hot off the press from @fertilitysterility! How many ovarian stimulation + IUI cycles is too many?

Objective: Study the clinical pregnancy rate (CPR) per ovarian stimulation with intrauterine insemination (OS-IUI) treatment cycle in patients with repetitive cycles up to a maximum of 8 cycles in those unable to proceed with in vitro fertilization IVF.

Methods: Retrospective cohort of 37,565 consecutive OS-IUI cycles from 18,509 patients were included in this study.

Results: Patients aged <35 years comprised 47.6% of the cohort. After adjustment for confounders, the mean predicted probability of CPR for cycles one to 8 was 15.7% per cycle. The mean predicted probability of CPR in aggregated data from cycles 2 to 4 was only 1.7% lower compared with cycle 1 as the referent. The mean predicted probability of CPR of cycles 5 to 8 was not significantly different compared with the referent (cycle 1).

The modeling of consecutive cycles suggested that the adjusted cumulative predicted probability of CP from OS-IUI continues to increase with each of the 8 successive cycles

Conclusion: Clinical pregnancy rates are satisfactory in up to 8 consecutive OS-IUI treatment cycles. These data are useful for counseling, especially in those patients for whom in vitro fertilization is not financially or ethically feasible.

Clapping on the beat: hard, apparently

Getting an egg, a sperm, and the uterus on the same beat: harder.

But we got you

Happy birthday @drblakeevans 🎂🥳 So nice to see you and your family spontaneously for a great Padres game ⚾️💛🤎 by @drblakeevans

Have you heard of a hydrosalpinx?

The presence of a hydrosalpinx decreases pregnancy rates by nearly half, and surgical treatment (salpingectomy or tubal occlusion) restores those rates toward levels seen in women without hydrosalpinx.

My closing slide dad joke to our @oureproductivemedicine REI fellows on the basics of the menstrual cycle…IYKYK

So if you don’t know: After ovulation, the ruptured follicle transforms into the corpus luteum — a temporary structure that produces progesterone (and some estrogen) to prepare and maintain the uterine lining for a pregnancy. It has a fixed lifespan of about 14 days unless it gets a specific signal. This is where the “two week wait” comes from as well.

That signal is hCG (human chorionic gonadotropin) - the hormone produced by an implanting embryo. hCG “rescues” the corpus luteum, telling it to keep producing progesterone so the pregnancy can continue to develop. This is what a pregnancy test detects (urine and/or blood).

If no embryo implants, no hCG signal is made. With no hCG signal to rescue, the corpus luteum degrades — a process called lutelysis - and shrinks into scar-like tissue called the corpus albicans (“white body”).

As it breaks down, progesterone and estrogen drop sharply, which triggers shedding of the uterine lining leading to menstruation.
