POSTS STORIES REELS

This is a photo fueled debrief post from a mid-jan week at the @blanketglacierchalet with the rad @capowguiding without cell service or wifi with 1 familiar face and 14 new faces. With the hilarity that ensued lapping free-refills on the pillowed glades, hoovering delicious calories back at the chalet, waddling to the outhouse in the middle of the night only to fall into the deep snow while gawking at the eye-and-heart-and-mind-opening number of stars, squirting coffee and ganache cake through a hole in your beeswax food wrap on the skin track in between laughs, failing to avoid massive seussian snow puffs falling on you while urinating in never-enough-cover, dilapidating the snow angel imprints with the sauna infused loose muscle sweats in -20C, deeply resting without acknowledgment of the chalet snores, or in the last vid - us making a three person hot tub off a huck-to-flat, all of it was another powerful example that human connection triumphs! We all go to these huts to ski, but the memories a month out are dominated by the interactions with the people even though the pow was self-righteous. It’s fun to prove it’s easy to be in an environment where you move your body for 7 hours a day in stunning scenery with new folks. This takes down walls, opens ears; the thoughts and smiles and convos on the skin track made the days and made the trip. I think a large part of the wonder of bringing folks together in the snowy woods for a week is acknowledgement of the natural coaxing by the shared experiences of being in nature which eases one into the present- just this rejuvenating and invigorating spiral of fun connection in the midst of lots of vert. Nice reflecting on the fact that your laughing and smiling and wahooing muscles got just as amazing a workout as your legs and diaphragm. Can’t wait for the next hut trip with strangers! by @mydocintow

Got to see some buddies and run in one of my favorite places in the world driving back from a fulfilling wilderness medicine job in New Mexico… and of course wanted to jump off the Phelps rock @grandtetonnps… and one of the things that made my heart fill up was watching this grandmom jump after her grandkids into Phelps lake!! I’ve never wanted to be a fun grandma more than this moment! by @mydocintow

To quote a mountrainiernps ranger from a dozen years ago… “mountaineering is a f*cking baller sport bros.” Pic 1 of an insane ice feature through which you can see the summit of Pumori en route to c1 through the Everest icefall. 2nd pic is of the same feature of you look midframe on the decent from the rotation… will be interesting to see if it’s still a bridge on the next rotation. #SummitPush (for my @asiantrekking clients as I just play doctor) This place is stunning!! #mountainmedicine by @mydocintow

Back from a great weekend at the @canadianwildmed conference in Whistler. 🏔️

Huge thanks to the organizers and sponsors for putting on such a solid event. After presenting at the last two conferences it was a treat to show up this year, catch up with old friends, meet a bunch of new awesome folks, and squeeze in a run and a swim in Lost Lake between sessions.

Grateful for the chance to present a literature review and theoretical framework on concussion and mTBI at altitude, and to lead a hands-on session on basic POCUS procedures in the backcountry- see fridge photo.

Best part, as always: learning from folks who are actually out there doing real wilderness, austere, remote, and low-resource medicine. This community keeps me sharp and is soo friendly!

Already looking forward to the next one. by @mydocintow

“FIVE-ISH BACKCOUNTRY POCUS NECESSITIES”

Saturday Session presented by Christian Dean DO, DiMM, FAWM, MS

Christian Dean DO, DiMM, FAWM, MS
Dr. Christian Dean is a sports medicine fellowship-trained physician working at the intersection of sports medicine, human performance, and international expedition medicine. Spent four days in Park City back in July at @inscyd_’s Coaching & Testing Camp — figured I’d share a bit of what I was up to.

The camp is about reverse-engineering endurance performance from the physiology up — VO₂max, VLamax, fat and carbohydrate combustion, lactate, and running, cycling, and swimming economy. This helped reframe how I think about programming — previously setting training zones off a percentage of a field test, and now directly building them from what an athlete’s metabolism is actually doing. Although I’m not a “coach,” it’s important as a sports medicine physician who helps athletes that I’m well versed in these conversations.
The part that stuck: two athletes with the same threshold can need opposite training — one long-and-easy, one short-and-hard — and the data tells you which.
It’s had me thinking about the mountaineering side too. Building toward an expedition or a big alpine objective, the same metabolic profiling sharpens the aerobic base and fat-oxidation you lean on when the air gets thin. Endurance metabolic testing has a place in a high-altitude mountaineer’s training block, just as it does for the ski patroller, bow hunter, pickleball player, and world-class nordic skier.
Big thanks to Sebastian Weber, founder of @inscyd_, @coachParkerSpencer_, head coach of USA Triathlon’s Project Podium, the @usatriathlon athletes who let us test them running at the UoU track, swimming laps in PC, and cycling on the climb up to @utaholympicpark, and to the other folks doing this great testing camp. Grateful for the four days of learning. Plenty to bring home to the new clinics… by @mydocintow

Have some changes coming to MyDocInTow so want to run through a lot of the stories and people interactions over the last couple years that have helped shaped what’s next. These are some pics from teaching with @worldextrememedicine for their biannual wilderness medicine course they do on the slopes of Mt Washington in New Hampshire. There’s some other amazing faculty which include Andy Miller, folks from @northeastern including Josh Merson @jrm_86, Jon with @thefieldcraftfarmer, and the ever rad @sarahspelsbergmd. Teaching these courses is such an invigorating time as I always come away with some new thought/knowledge/skill and always come away with new friends and colleagues… and some HomeGoods shopping in North Conway. by @mydocintow

Of the many hats I wear, I provide medical direction for many different programs. One of them is an amazing wilderness program of a rad (I’m a biased alum) high school in Dallas. The St Mark’s School of Texas’ annual backpacking trip to the Pecos Wilderness east of Santa Fe looked a little different this year due to Jack’s Creek campground construction and closure forcing a Cowles based airbnb as the basecamp… and the Frijoles fire popping up from lightning on the NW border of the Pecos while the trip was underway. Luckily the kids were able to evacuate and get back home safely. Grateful for those wild land firefighters that are still fighting that fire! And so nice to get some added time in Santa Fe to see family and catch an Opera and @santafeindianmarket! by @mydocintow

Chasing winter to New Zealand with the amazing women’s @ustechteam @usskiteam at their @roundhillnz camp was sweet! Truly honored to be invited to help keep these rad (and funny) women healthy and firing — thank you to the athletes, techs, PT, and coaches for having me! Excited for these fast ladies to start racing in a few months. 🇺🇸⛷️ And really nice to hike in Aoraki (w some beautiful Aoraki pics) and catch up with some great friends in Wanaka too! by @mydocintow

So much love to the folks that helped me excel through my sports medicine fellowship- truly something I thought about doing since second year of residency a decade ago and wild to have had this clinical curiosity for so long and now have this fun medical learning experience and professional chapter behind me. I’ve missed a bunch of events and memories this last block from July ’24 to June ’25, as one does when you work largely 6 days a week. But doing it all and burning the candle at both ends is another board cert I have. Awesome to finally learn more nuances of msk exams and both diagnostic and therapeutic ultrasound procedures and insights on human performance. The biggest joy of the year was the people that were overseeing my learning, the people that were helping in clinics, the MAs, ATs and other students/ learners. All this with some rockstar mentors like Efren Caballes, Dan Haber, Rachel Brewer, Andrew McBride, Kate Reitz, as well as my PDs Erin Morine and Nico Gamble, and so many other docs that spent so much of their own time with me.
