If you have histamine intolerance or MCAS, are peptides off the table?... Recently on an Ask Me Anything episode of The Art of Being Well, Brittany asked whether it’s safe to try peptides like BPC-157 if you have MCAS or histamine intolerance. It’s a fair question, because people with mast cell activation syndrome can react poorly to even basic supplements. Histamine intolerance is often described as a “pseudoallergy.” It isn’t a true IgE allergy. Mast cells release histamine through receptors like MRGPRX2, and they don’t know how to calm back down. How reactive someone is depends on what’s underneath. Mold, chronic Lyme, SIBO, gut dysfunction, trauma. That’s why there’s no one-size-fits-all answer. Here’s what I see clinically: It’s rarely the peptide itself causing a flare. More often it’s the fillers, the purity, the sourcing, or the dose. So start low and slow, and work with a practitioner. The stack we use in this situation includes BPC-157, KPV, TB-500, Thymosin Alpha-1, and VIP for SIRS-type presentations. And the stat that surprised even me… A 2025 case series in the American Journal of Medical Sciences looked at 47 people with MCAS who were put on a GLP-1, and 89% saw a remission of their MCAS symptoms. It’s one small case series, so it isn’t a recommendation, but it’s interesting. You’re essentially modulating the gut, and the gut is where about 75% of the immune system lives. Mast cell support like quercetin, luteolin, stinging nettle, DAO, and vitamin C can pair with this, and the nervous system absolutely matters as well.
