Obesity doesn't change the shape of the human skeleton, but it dramatically changes the workload those bones carry. The same frame that supports a lighter body ends up bearing far greater force, and over years, that added strain leaves its mark on the joints.
The math is striking. Orthopedic specialists estimate that for every extra pound of body weight, the knees absorb roughly four pounds of additional pressure with each step, and even more when climbing stairs. Carrying an extra 50 pounds, then, can translate to around 200 pounds of added force pounding through the knees every stride. That relentless loading falls hardest on the weight-bearing joints, the knees, hips, ankles, and lower spine.
Over time, the consequences compound. The smooth cartilage cushioning the ends of bones wears down faster under the excess load, a leading driver of osteoarthritis, and studies find people with obesity are far more likely to eventually need a knee replacement. The strain doesn't stop at the joints, either. Extra abdominal weight shifts the body's center of gravity forward, straining the muscles and discs of the lower back and altering posture and gait. There's also a chemical dimension, as fat tissue releases inflammatory signals that can further accelerate joint breakdown beyond mechanical wear alone.
