I had an employee at Microsoft with “gold standard” health insurance on a Kaiser plan who was unfortunately hit in a motorcycle accident — at the ICU before they would cast his broken arms, they made him sign for financial responsibility because the hospital he was ambulanced to was out of network and Kaiser declined coverage. Only because I elevated it internally and Microsoft fought it did Kaiser cover his emergency care. That should also be illegal.

This is absolutely insane. I guess at the first sign of anaphylaxis go to the ER and not urgent care. What the hell are we doing?
