$If your labs came back “normal” but you still don’t feel like yourself, this could be exactly why. Most standard panels check LDL, glucose, and maybe TSH, and call it a day. That’s a fraction of the picture. A complete menopause workup goes deeper into your hormones and also looks at your cardiovascular risk and metabolic changes. These are the markers that often explain the belly weight, rising cholesterol, and “same habits, different body” changes that basic labs miss. I can’t count how many patients come to me after being told everything is fine, only to find real, treatable imbalances once we run the full panel. Follow @renata.gazzi. Your mammogram was normal. But do you know your breast density?

About 40% of women over 40 have dense breasts. Dense tissue and tumors both look white on a mammogram, so cancers can be harder to see. And density itself is an independent risk factor for breast cancer.

What I want every woman to know:

1. Know your density. Your report should say if your breasts are fatty, scattered, heterogeneously dense or extremely dense.
2. Ask about additional imaging. Depending on your density and risk, ultrasound or MRI may be appropriate.
3. Understand your personal risk. Family history, genetic variants and prior biopsies can all shape your screening plan.

The future isn’t one-size-fits-all. It’s precision medicine. The WISDOM Study is researching screening guided by individual risk, including density and genetics.

Your next mammogram matters. Understanding what it may miss matters too. Save this and send it to a woman who’s due. by @renata.gazzi

These medications can support appetite regulation, weight loss, blood sugar control, and overall cardiometabolic health. But they work best when they’re part of a bigger picture, not treated like a shortcut.

Nutrition still matters.

Strength training still matters.

Sleep, stress, hormones and consistency still matter.

The goal isn’t just to lose weight. It’s to create a plan that supports your health, your metabolism and your long-term results.

And most importantly: there is no one-size-fits-all approach. What works for one woman may not be the right strategy for another.

What your hormonal symptoms are telling you:

→ Belly weight gain that won’t budge, no matter what you change? Often tied to high insulin when cells stop responding to insulin efficiently, your body stores more fat, specifically around the abdomen.

→ Adult acne, especially around the jawline or chin? Can point to elevated testosterone, which increases oil production and clogs pores in a very specific pattern.

→ Anxiety or irritability that feels disconnected from anything specific? Cortisol is a common driver. It directly activates your brain’s fear and stress center, keeping you in a low-grade “on-alert” state even when nothing is actually wrong.

→ Low libido? Frequently linked to low estrogen or low testosterone, both of which play a direct role in desire and arousal.

→ Struggling to fall or stay asleep, even when you’re exhausted? Low progesterone is often part of the picture. It has a calming, sedative-like effect on the brain, and when it drops, that effect goes with it.

A quick caveat: these are patterns I see constantly in practice, not a diagnosis. Hormones interact with each other, and the same symptom can have more than one cause, which is exactly why testing matters more than guessing.

If two or more of these sound familiar, it might be worth asking for a full hormone panel instead of treating each symptom as its own separate problem.

Nobody hands you a manual when your metabolism starts changing. Most of us just keep doing what used to work and then wonder why it doesn’t work anymore.

Here’s what’s actually happening: as estrogen declines, insulin sensitivity drops, fat storage shifts toward the abdomen, and muscle becomes harder to maintain. None of these happen in isolation. They happen together, which is exactly why “eat less, move more” stops being the full answer.

The response that actually works in this phase: building muscle, eating enough protein, and keeping blood sugar steady.

If you’ve been doing everything “right” and still not seeing results, this is probably why.

Today is World Menopause Day, and this is a conversation we still need to have: is menopause a disease?

No. But here’s the nuance that gets lost in that question.

Menopause itself is a natural biological transition. Every woman who lives long enough will go through it. In that sense, calling it a “disease” medicalizes something that isn’t inherently pathological.

But here’s where it gets complicated: the hormonal changes behind menopause have real, measurable effects on your body. Declining estrogen affects sleep, mood, bone density, cardiovascular risk, and metabolism. Those aren’t “just symptoms to push through”. They’re physiological changes with long-term health consequences if left unaddressed.

For decades, the “it’s natural, just deal with it” framing has meant women were undertreated, dismissed, or told their very real symptoms were “all in their head.” At the same time, treating menopause purely as an illness can make women feel broken for going through something completely normal.

The truth lives in the middle: menopause is a natural transition that still deserves real medical attention. You don’t need a diagnosis to deserve care, and you don’t need to accept suffering just because it’s “natural.”

This is exactly the conversation I have with patients every week.

3 pillars, zero trends: daily walks, strength training, and VO₂ max work the habits with the strongest evidence behind them for women 40+.
