#Avascularnecrosis (AVN) is a common chronic complication of sickle cell disease (SCD) that affects about 10% to 50% of patients. The rate increases significantly with age; up to 50% of adults with the severe HbSS genotype develop the condition by the age of 35.

The impact and prevalence of AVN within the SCD population highlight several key factors:Prevalence Rates: Imaging studies (like MRIs) detect “silent” or symptomatic AVN in up to 41% of SCD patients.Affected Joints: The hip (femoral head) is the most commonly affected area (about 86% of cases). Other frequent sites include the shoulder (humeral head), knees, and feet. In about 50% of patients, the damage occurs bilaterally (in both joints).Risk Factors: The likelihood of developing AVN increases with older age, more frequent vaso-occlusive pain crises, a history of acute chest syndrome (ACS), and concurrent vitamin D deficiency (VDD).

Impact & Management: AVN is a major cause of chronic joint pain, impaired mobility, and disability in SCD patients. While early, mild cases can be managed with conservative therapy (like physiotherapy and pain control), advanced stages often necessitate joint replacement (arthroplasty) surgeries.
