Maravillosa historia y muchas gracias a @primerimpacto @univision @pamelasilva . Gran esfuerzo de equipo de 12 especialidades médicas. Madre, bebé y familia increíblemente fuertes. Malformación cerebral vascular tratada in utero. Cuarto caso reportado. Después del tratamiento, el bebé tuvo una resolución de la insuficiencia cardíaca y pulmonar y está cumpliendo con los hitos cerebrales. Maravilloso para que el bebé regrese a los 7 meses para darle un apretón de manos o dedo.

Wonderful story and many thanks to @univision @primerimpacto @pamelasilva. Great team effort by 12 medical specalties. Incredibly strong mother, baby and family. Vascular brain malformation treated in utero. Fourth reported case. Following treatment the baby had resolution of heart and lung failure and is meeting brain milestones. Wonderful for baby to return at 7 months for a finger / hand shake.

Tough Father’s Day for Mr Vera who had a seizure and bleed in the brain from a rare vascular malformation and was transferred to an ICU in another country. He had various opinions from surgeons that did not feel he had a treatment option. Although we commonly perform awake brain surgery for tumors in important areas, awake brain surgery for vascularar malformations is rare. Mr Vera presented with bleeds in the brain and seizures. His vascularar malformation was next to areas of the brain controlling speech. Due to his high risk of bleeding again, surgery was the best option. During awake surgery he can talk to us through the removal of the malformation allowing us to test key speech areas for preservation. Additionally we can stimulate various areas of the brain to increase the chances of removing seizure foci alterations with the vascular malformation. Thankfully through this minimally invasive approach he was home two days later and back to his amazing family. As on of the few overall centers of excellence in the United States for the angioma alliance, allows us to bring clinicians together from many areas to improve clinical outcomes and research for u common vascular malformations. Thanks to various news stations for putting together this great story. https://www.local10.com/sponsored/2024/01/24/awake-brain-surgery-allows-dad-to-play-another-day/ @abc

Almost never wear a white coat. Scrubs / pajamas seem to suffice. I put one on to cross over to the give neurosciences lecture for medical students. Found these 3 tokens I used to always have in my pocket:
1. Air Force coin. Love taking care of veterans. Such tough people. I treated his two aneurysms surgically. He said the Air Force gives out these coins when you have mastered something and they trade them to pay for drinks, favors, thank you’s.

2. Building block my son gave to me to prepare me for work when he was 3. He said it would help fix people.

3. Bracelet son made me at age 5 for good luck.

Finish all my talks with a picture thanking him for his help! by @starke_neurosurgery

3 days in a row, I operated through the day, went home to see my son, take our evening walk, and put him to bed, and return to operate through the night.

I was driving him to school the next day before starting a normal work day, and we had a similar conversation.

Son: “Dad where do you get your strength from?”

Dad: “it comes from you..my goal every day is to be a little better than the day before and be a good role model for the important people around me…I will be there for you and good things will happen today…”

Son: “What do you do when you are tired?”

Dad: It’s important to rest, but my littlest patients show me that we can be so much stronger than we imagined…” by @starke_neurosurgery

Innovation in stroke thrombectomy isn’t just about bigger catheters. It’s about smarter aspiration. Thanks so much to Journal of NeuroInterventional Surgery (JNIS) for highlighting our work & to National Institute of Health for supporting our research.

Adaptive Pulsatile Aspiration (APA™) improves clot ingestion, first-pass reperfusion, and revascularization while reducing distal embolization.

These encouraging preclinical findings reinforce our belief that the next generation of thrombectomy innovation will come from rethinking how aspiration interacts with the clot—not simply increasing catheter size.

👏 congragulations to the outstanding research team: Nadia McMillan, Joshua Hanna, Sai Sanikommu, Pedro Bartkevitch Rodrigues, Jayro Toledo, Ahmed Abdelsalam, John Thompson and the Von Vascular team.

Our sincere appreciation to the editors, reviewers, and team at the Journal of NeuroInterventional Surgery & National Institute of Health for their commitment to advancing scientific discovery and for highlighting this important research.

This is an exciting step forward, with clinical validation still ahead!

Through research and technology we can improve outcomes for our patients.

Only takes a moment to understand what is waiting for someone at home or what they are most excited about when leaving the hospital. 20 years ago, I could not have imagined brain surgery and then home the same or next day. Many thanks to our strong patients, their families, and our excellent team! by @starke_neurosurgery

Our systematic review and meta-analysis examined whether modifiable cardiovascular risk factors and commonly used therapies are associated with hemorrhagic events in cerebral cavernous malformations (CCMs).

Across 22 studies involving 7,331 patients, smoking was associated with higher odds of hemorrhagic presentation, while antithrombotic therapy was associated with lower odds. These findings are preliminary: most analyses concerned presentation at diagnosis, and they do not establish cause and effect or predict future hemorrhage.

More prospective research is needed before these results can guide treatment decisions.

Excited to share our latest publication in *Neuromodulation: Technology at the Neural Interface*!

“Invasive and Transcutaneous Vagus Nerve Stimulation for Upper-Limb Recovery After Stroke: A Systematic Review and Meta-Analysis of Randomized Controlled Trials”

Across 11 randomized controlled trials involving 402 patients, vagus nerve stimulation paired with rehabilitation significantly improved upper-limb motor recovery after stroke. Benefits were observed with both invasive and transcutaneous approaches, while transcutaneous VNS may offer a more accessible and scalable option that avoids surgical implantation.

Grateful to @starke_neurosurgery and all our coauthors for their mentorship, collaboration, and dedication to advancing stroke recovery and neurorehabilitation.

ANNUAL NEUROSURGERY REGATTA: For the last 12 years, attendings get their boats together to take out residents & fellows and their families. Starts out with wake surfing before getting all the boats together. Amazing to see people try something for the first time or get better through years. Although we had one rainy day, it brought people closer together. Many thanks to all the folks who pitch in each year. by @starke_neurosurgery

Super proud of this latest publication from our group in *Stroke: Vascular and Interventional Neurology*:

**”Beyond the Catheter: Contemporary Aspiration Pump Systems in Neurointerventional Stroke Therapy”**

This review explores the evolution of aspiration thrombectomy technology, from early syringe-based aspiration techniques to modern continuous, cyclic, and adaptive pulsatile aspiration systems. We discuss the engineering principles, clinical evidence, and future directions shaping mechanical thrombectomy for acute ischemic stroke.

As stroke intervention continues to evolve, understanding the technology behind clot retrieval is critical to improving procedural success and patient outcomes.
