What does the Community Advisory Council mean to the people who are part of it? Meet Yvette Smith, a dedicated member of our Community Advisory Council (CAC), who has been serving for the past year. In this reel, she shares what being part of the CAC means to her, and why it matters. Our Community Advisory Council is made up of community members who help guide and strengthen the work of our Family Success Center. CAC members share ideas, give feedback, and make sure programs reflect the real needs, voices, and experiences of families in our community. This group helps us stay connected. It keeps us accountable. And it ensures the services and events we offer are shaped by the people they’re meant to support. We’re grateful for members like Yvette who show up, speak up, and care deeply about making our community stronger. If you’re curious about getting involved with the CAC or want to learn more about how it works, send us a message or drop a comment below. We’d love to connect. #FamilySuccessCenter #BellevueCommunity #WashingtonDC #CommunityofHopeDC #dccommunity

Transportation, cost, and stigma still prevent many people from seeking care. Community-based healthcare closes those gaps. Equity requires intentional action. BRINGING HEALTHCARE TO NEIGHBORS EXPERIENCING HOMELESSNESS Roof Above’s Day Service Center (DSC) provides lunch and basic services such as showers and laundry 365 days a year, and also helps neighbors connect to resources including shelter and housing. We serve hundreds of people every day. C.W. Williams Community Health Center’s goal is to extend the reach of affordable healthcare to everyone, especially the most vulnerable among us. Their clinics on our campus do just that, and we are proud to partner and expand the services that we offer our neighbors!#endhomelessness #healthcareforall #housingishealthcare
We showed up! ⠀ ⠀ Care at Home Day at the Capitol brought providers, leaders, and advocates together with a shared focus, making sure care at home is recognized, supported, and prioritized.⠀ ⠀ These conversations matter. ⠀ ⠀ They impact funding, standards, and the level of care people receive every single day.⠀ ⠀ Home care is a critical part of the healthcare system.⠀ ⠀ We are proud to stand alongside others pushing this industry forward and making sure our voices are heard where it counts.⠀ ⠀ @hcaoa @cthealthcareathome⠀ ⠀ #ncehomecare #cthomecare #homecare #careathome #changingthestandardsofhomecare #ourfamilytakingcareofyours #welovewhatwedo #homecare #nowhiring #fundhomecare #reels

We had a great time attending the @n10_phl Hunting Park East Tioga Community Impact Expo So many wonderful organizations were in the room sharing resources and doing important work for families across Philadelphia. We connected with some amazing partners and cannot wait to share these resources with our Oshun families. Community collaboration like this makes a real difference. 💛 #Community #MaternalMentalHealth#oshunfamilycenter #CommunityImpact

Chris grew up feeling like an outcast, without much support or guidance. After making a few bad decisions, he found himself incarcerated and unsure of what life would look like next. When Chris connected with EAC Network’s Health Home Care Coordination program, he began receiving critical support, including food assistance, transportation, and bus tokens, to help meet his daily needs. With consistent care and encouragement, Chris is now regaining his independence one step at a time. His journey shows how compassionate support can help individuals rebuild their lives and move forward with dignity and hope. 🎥 Hear more of Chris’s story at the link in bio. #healthhomecare #carecoordination #secondchances #reentrysupport #nonprofitwork #socialimpact #equity #communitysupport #incarceration #longislandnonprofits #newyorknonprofits #missiondriven #humanservices #communityequity #impactfulwork
Kameisha Heppard, Director of Homeless Services for the City of Columbia, is introducing a new partnership with Health Management Associates, Inc. to strengthen services and support for our unsheltered neighbors. Through this comprehensive community assessment, the consulting team will work alongside service providers, stakeholders, and individuals with lived experience to identify barriers to housing stability, evaluate mental health and substance use resources, promote trauma informed care practices, and build a clear, data driven roadmap for long term solutions. This effort will result in a strategic plan with measurable outcomes to better align resources, enhance service delivery, and create sustainable pathways from homelessness to stability. We are calling on grassroots agencies and organizations that work directly with unsheltered individuals to participate in upcoming focus groups on March 17 and March 18, 2026. Your insight will help shape more effective, community driven solutions. To participate, email [email protected] with the subject line “Homeless Services 2026.” For more information, visit the link in our bio. Your voice matters. Together, we can make a meaningful impact. #TogetherWeAreColumbia
