Skin cancer, particular melanoma can look like normal spots on the skin. When you look at these two spot, which one did you think was melanoma? Spot 1 or spot 2? The way to tell is by looking at the zoomed in view of the spots. Dermoscopy (zoomed in view) allows your dermatologist to discern what is cancerous and what is safe. But, they need to use this tool (a dermatoscope), and know what to look for. Compare the two spots, and see how different they look on the inside. Spot 1 is melanoma! At first glance, we recognize this as a melanocytic lesion. There are no clear features suggestive of solar lentigo or flat seborrheic keratosis, such as the jelly sign or fingerprint-like structures. On dermoscopy, we can identify focal pigment network areas along with irregular homogeneous brown pigmentation, resulting in structural asymmetry. Although this is not a structure-rich lesion, melanoma cannot be excluded. This diagnosis lies within the gray zone between nevus and melanoma. In fact, cases like this may represent early melanomas arising on photodamaged skin in older patients. The key point is: for us, dermoscopers, this is not a lesion to overlook without suspicion of malignancy.
