👨⚕ Patient Profile:
o 40-year-old male with a history of hypertension.
o Presented with dyspnea on exertion (DOE), NYHA class III.
• 🩺 Key Echocardiographic Findings:
o Large mass at the LV apex with central lucency 🌀.
o Regional wall motion abnormalities (RWMA) detected 📉.
o Severely reduced LVEF: 19% ❗.
• 🧐 Diagnostic Dilemma:
o Possible LV thrombus 🧊 due to stasis and low EF.
o Suspicion of intramyocardial dissection hematoma 🩸, a rare complication.
o Central lucency suggests liquefaction necrosis or hemorrhagic transformation.
• ⚖ Why It Matters:
o LV thrombus ➡ Risk of systemic embolization, requires anticoagulation 🩹.
o Intramyocardial hematoma ➡ Potentially catastrophic, may need surgical intervention 🚨.

This 78-year-old male with known CAD, hypertension, and diabetes mellitus now presents with new-onset exertional dyspnea, easy fatigability, and sweating, which is a high-risk presentation given his background.
His previous evaluation shows:
• LVEF ~35% (HFrEF)
• RWMA positive → ischemic cardiomyopathy
• Mild MR and TR
• Prior chest pain managed conservatively

65year old female presented with Giddiness followed syncope .ECG showing Complete heart block Immediate TPI was inserted and patient doing well,Echo No RWMA with Moderate TR/PAH ,Normal LV systolic function ,LVEF-54%.High sensitive TROP I was positive.Patient rythm after 4 days of TPI is still in CHB.Hence,Decision taken to do CAG followed Permanent pacemaker implantation.Interestingly CAG showed coronary cameral fistula draining into Right ventricle from RCA.Later successful PPI VVIR implantation done.Advised Transcatheter coil or vascular plug during follow up .Mostly coil may be used in this case as it is more tortous than linear #heart #heartmemes #heartattacksurvivor #coil
