Gluteal enthesopathy with co existing axial spondyloarthropathy
Axial spondyloarthritis (axSpA) is an inflammatory condition of the axial skeleton associated with significant pain and disability.
Enthesitis is a characteristic clinical manifestation of spondyloarthritis (SpA) and a key feature of axSpA.
In axSpA, enthesitis can result from mechanical or other inflammatory stressors, chronically inflaming entheses in genetically susceptible individuals. Peripheral enthesitis in PsA has been associated with the presence of human leucocyte antigen (HLAB27).
Interestingly this patient had previously had X-rays which were clear. This subgroup labeled non-radiographic (nr)-axSpA often involved a higher percentage of women where less structural damage is evident.
Where in this case the patient had lower back symptoms, they also had features of gluteal tendinopathy but didn’t fit the norms of this patient making enthesopathy a more likely differential. The clinical presentations are similar but the reason behind the onset is quite different.
How do you manage it? Educate. Advice. Keep them moving. Get the tendons stronger. Make the patients understand what is going on how to manage it. Support in building physical and psychological resilience. Get input from MDT if needed.

Fatigue hits differently when you live with spondyloarthritis.Fatigue isn’t just being tired — it’s one of the most challenging and misunderstood symptoms of he disease.
In our newest episode of Spondycast, Dr. Susan Murphy introduces the SAA-funded RENEW Study, a groundbreaking research initiative from the University of Michigan focused on fatigue, energy management, and resilience in people living with spondyloarthritis.
If you’re navigating fatigue and wonder what science says about energy management and how to strengthen resilience, this episode is for you.

⬇️ If you have 4 or more criterias then it could be very interesting to push the investigation further …💉🩸
✅So which one is the 5th criteria ?
•When 𝘁𝗵𝗲 𝗯𝗮𝗰𝗸 𝗽𝗮𝗶𝗻 𝗶𝗺𝗽𝗿𝗼𝘃𝗲𝘀 𝘄𝗶𝘁𝗵 𝗺𝗼𝘃𝗲𝗺𝗲𝗻𝘁
🔍These criterias are helpful when you’re looking for a systemic inflammation like in the Axial Spondyloarthritis (AxSpa).
These criterias also have differents odd values, which can place maybe more emphasis in your cluster.
AxSpa is not a « real » serious spinal pathology, but the treatment will be different and needs referring.
It also gives more meaning to a patient pain experience.
It probably contraindicates some techniques, and treatment rules.

Fatigue is such a complex topic when it comes to axial spondyloarthritis (AxSpA) or ankylosing spondylitis (AS). It’s not just tiredness—it’s so much more than that. The reasons behind it can vary, whether it’s linked to inflammation or something else entirely. While I won’t go too deep into the causes here, let’s talk about two simple yet powerful ways to help manage it.
🔥 Optimize Your Sleep
Even when you get a full night’s sleep, fatigue can still linger—it’s not the same as being tired. But improving your sleep quality can make a real difference for many people. Curious how? Check out our blog for specific tips on How to Sleep with Ankylosing Spondylitis.
✨ Restorative Practices Are a Game-Changer
If you’re struggling with fatigue, techniques like yoga nidra, restorative yoga, and calming breathwork can work wonders. These tools have helped countless members of the Yoga for AS community, including me, feel more energized and less weighed down by fatigue.
