Functions of kidneys…. 1. Excretion of waste products 2. Regulation of water & electrolytes 3. Acid–base balance 4. Blood pressure regulation 5. Stop overcomplicating hyponatremia. Think in 2 steps: 1. Is ADH on? → urine osmolality 2. Why? → urine sodium Patterns: • Low urine osm → excess water intake • High urine osm + low urine Na → low effective volume • High urine osm + high urine Na → SIADH But always rule out: • Hypothyroidism • Adrenal insufficiency If you’re not checking urine studies, you’re guessing. #nephrology #nursing #meded #np #physicianassistant

Proteinuria (protein in the urine) is NOT from the protein you eat! What is proteinuria? Normally, there is no protein in your urine because the filter in your kidney is too tight to allow a large molecule like protein through When the filter in your kidney is damaged, protein in your urine shows up. It’s one of the first signs of damage not just to your kidneys, but to your blood vessels as well! What causes it? Diabetes (goal hgb A1c is <7) NSAIDs Infections Inflammation/Autoimmune conditions Viruses Very high blood pressures Abnormal blood proteins (multiple myeloma) How do you know if you have proteinuria? Signs: swelling, foamy urine, shortness of breath Lab tests: urinalysis (although this will only show albumin), urine protein creatinine ratio (this shows all proteins), urine albumin creatinine ratio To providers: Please to refer to nephrology if a patient has proteinuria and you don’t have a clear reason (like NSAID use or hgb A1c of 13)! You can recheck it in 3 months to see if it was an off reading (make sure the patient hydrates) Labs to check while waiting for referral: hgb A1c, hep panel, ENA (I prefer to check for most autoimmune conditions at once), ANA, ANCA, serum/urine protein electrophoresis/immunofixation, PLA2R, anti-GBM Medications that improve proteinuria: ACEi, ARB, MRA (like spironolactone), and SGLT2i…new studies are suggestive of GLP-1s as well Please do NOT stop eating protein if you have proteinuria, most of my patients are not close to eating enough protein already. I want you to be strong, not frail, so please get your protein in (1g per kg body weight) Instead, see your local kidney doctor, stop taking NSAIDs, work on getting your blood sugar under control. You got this! #doctorsofinstagram #kidneyhealth #internalmedicine #foamed #nephrology
🎉 From Kidney Disease Stage 3A ➡️ Stage 2 Here’s what actually made the difference: 👇🏼 1️⃣ He stopped Googling the “renal diet” …and finally understood how blood pressure, blood sugar, and cholesterol impact his kidneys. 2️⃣ He learned to read his labs For the first time, he understood what his numbers meant—and why they were changing. 3️⃣ He connected the dots With a personalized plan, he went from guessing to knowing what supports his kidney health. 〰️〰️〰️ When he first came to me, he felt lost. His labs weren’t explained, and online advice just made things more restrictive and overwhelming. 🍾 A few months later? His GFR improved by 25 points. But more importantly—he feels confident, clear, and in control of his choices. His words: “No one ever explained my labs before… now I finally know what to do.” If you’re worried about your numbers getting worse, don’t wait. 🎉 Watch my FREE Kidney Nutrition Webinar where I break down the proven roadmap I use with my clients to help them improve their kidney function. I’ll walk you through how to improve your labs, eat with confidence, and stop second-guessing everything. 🔥 Comment “WEBINAR” and I’ll send you the link! . . . #kidneys #lowsodium #nephrology #renal #chronickidneydisease
2 years… on dialysis… unnecessarily 😳 This patient was told she needed lifelong dialysis. But when she came to Dr. Shoeb Khan, the truth was revealed — it was AKI (Acute Kidney Injury), not permanent kidney failure. 👉 Dialysis was STOPPED 👉 Permcath REMOVED 👉 Now she is living dialysis-free ⚠️ Important message: Not every dialysis patient needs lifelong treatment. If your dialysis started due to AKI, regular follow-up is MUST. Many patients continue dialysis just because no one reassessed them. 💡 Always monitor: • Creatinine • Potassium • Urine output • Weight Don’t continue dialysis blindly. Ask questions. Seek expert opinion. 📍 SAP Kidney Center Appointment no. 8075288559 🔁 Share this — you might save someone from unnecessary dialysis. #Dialysis #KidneyRecovery #AKIvsCKD #Nephrology #KidneyHealth
👍CKD is all about potassium economics. Healthy kidneys excrete excess K⁺. But Diseased kidneys accumulate it → arrhythmia risk. 🍎Apple, papaya, pineapple → safer because they deliver less potassium per calorie. Dose matters more than the fruit itself. #salubriousbyshreshta #health #kidneys #nephrology
