📝 Como calcular o volume do derrame pleural através da Fórmula de Balik: 1️⃣ Paciente em decúbito dorsal, com cabeceira elevada a cerca de 15-30 graus 2️⃣ Utilizar transdutor convexo (ou setorial) em sentido longitudinal, inicialmente, em linha axilar posterior, na região tóraco-abdominal. Rotacionar o transdutor (sentido horário, no hemitórax esquerdo e sentido anti-horário no direito). O artigo original de Balik utilizou a insonação transversal. Na nossa experiência, a insonação oblíqua, insonando através do espaço intercostal, ajuda a eliminar a sombra acústica das costelas e se faz adequada para mensuração do líquido. 3️⃣ Congelar a imagem no final da expiração 4️⃣ Ativar a tecla Caliper (ou Paquímetro) e traçar uma linha entre pleural visceral e parietal (região do diafragma) 5️⃣ Multiplicar a distância (em cm) por 200. Ou, se em mm, multiplicar por 20. ✅ Ex: distância interpleural de 2,45 cm x 200 = 490 mL de líquido pleural 💡Vale a pena refletir que nem sempre a avaliação do volume exato do derrame pleural é necessária, mas uma avaliação quantitativa ou semi-quantitativa pode ser importante para aqueles menos experientes em PoCUS, para tomada de decisão e correlação ecográfico-clínica. Algumas das situações que podem tornar a mensuração mais sujeita a erros: ⚠️ derrames loculados ⚠️ pequenos derrames ⚠️ deformidade torácica ⚠️ posicionamento inadequado do transdutor Obs: o estudo original de Balik foi realizado com pacientes adultos e sob ventilação mecânica. Scopriamo la POCUS! 🫁 Tosse? Sarà bronchiolite, bronchite, polmonite… ??? Ecco cosa succederà nei prossimi anni. Negli ambulatori di Pediatria e Medicina Generale vedrete sempre più spesso uno strumento considerato un vero e proprio “prolungamento del fonendoscopio” 🩺 All’inizio lo usavano solo pochi pionieri, ma dalla pandemia Covid in poi si è compresa la sua utilità: la POCUS (Point-of-Care Ultrasound), l’ecografia eseguita direttamente al letto del paziente. Oggi disponiamo di strumenti di piccole dimensioni, praticamente tascabili praticissimi in pediatria 👍🏽 Non sostituisce sempre la tradizionale Rx 🩻 ma è completamentare e soprattutto è innocua e ripetibile, perché non “irradia” il paziente, si esegue sul lettino o in braccio ai genitori e non è minimamente invasiva. Si realizza giocando! Vi chiederete perché non è ancora diffusa a livello capillare. Per due ragioni: questo piccolo strumento ha un costo molto elevato che deve coprire il singolo Medico (il SSN non lo fornisce), inoltre serve tempo per formarsi. Ma pian piano si diffonderà sempre di più a livello capillare. ETA: Here’s the answer! ECHO: showed a classic D-shaped septum on parasternal short axis view, with Right ventricle dilation impinging on Left ventricle filling. What does that clinically mean? Something SUDDENLY created a massive right ventricle pressure overload, most likely attributable to a massive Pulmonary embolus (PE) Treatment: what did they give? tPA (tissue type plasminogen activator), a “clot busting” or “thrombolytic” medication. It is massively effective for breaking clots - but sometimes “too effective” in that it can break up the clot you want (like in this case, the blood clot in the lungs), but ALSO break other clots in other places and lead to massive bleeding, including in the brain or other areas. That’s why it’s a drug that when we use, we have to extremely carefully weigh the risks and benefits. In this case, the patient was in cardiac arrest, and emergently treating the PE was the only way to help them survive. . . . What do you think? (1) What medication did they give?
