👁️ A retinal tear isn’t something to ignore! Think of the retina as the wallpaper lining the inside of your eye. A retinal tear happens when that delicate tissue develops a small rip. If left untreated, it can allow fluid to slip underneath the retina, leading to a retinal detachment—a vision-threatening emergency. ⚠️ Symptoms to watch for: ✨ Sudden flashes of light ✨ A shower of new floaters ✨ A curtain or shadow in your vision ✨ Sudden decrease in vision 👥 Who is at higher risk? • High myopia (nearsightedness) • Age-related posterior vitreous detachment (PVD) • Eye trauma • Previous retinal tear or retinal detachment • Family history • Prior eye surgery (especially cataract surgery) 🔍 How is it diagnosed? A comprehensive dilated retinal exam is the gold standard. Your eye doctor may also use ultra-widefield retinal imaging (Optos), OCT, or an ultrasound when indicated to better evaluate the retina. 🩺 Common types of retinal tears: • Horseshoe (flap) tears • Operculated tears • Atrophic holes • Giant retinal tears 💡 The good news? Many retinal tears can be treated with laser before they progress to a retinal detachment. If you ever notice a sudden onset of flashes, floaters, or a curtain over your vision, seek an urgent dilated eye exam. Here’s what that actually looks like in real life: At -1.00, you might still pass a driver’s test without glasses—but street signs aren’t crisp.� At -2.00, your clear vision stops around 20 inches—reading is easy, but the TV is blurry.� At -3.00, even faces across the dinner table lose detail. As the numbers go up, your world gets smaller: At -4.00, clear vision is just 10 inches away.� At -5.00, you’re reaching for glasses the moment you wake up.� At -6.00, anything beyond 6 inches is a blur. This happens because the eye grows too long, so light focuses in front of the retina instead of directly on it. It helps turn light into signals that are sent to the brain so you can see. But when the retina starts pulling away from the back wall of the eye, it is called retinal detachment. 🚨 Sudden symptoms can include seeing lots of new small spots or shadows, flashes of light, or a dark curtain/shadow coming across your vision. ⚠️ Retinal detachment is an eye emergency. If these symptoms suddenly appear, seek urgent eye care.
Are those fish eggs? Caviar? Those are PFCL bubbles! This was a case of a giant retinal dialysis with total retinal detachment closed with dual short tamponade (2/3rd PFCL +1/3rd SO). The patient will be now taken up for PFCL + SO removal followed by reSOI PFCL as opposed to Silicone oil is heavier and settles down. Whereas SO bubbles float upwards forming hyperoleon. Due to low interfacial tension, PFCL breaks up easily and forms these bubbles called “fish eggs”. Here, it has migrated into the anterior chamber.

You might not realize you have it until it’s too late. 👁️Amblyopia: Also known as ‘lazy eye,’ the incomplete development of the nerve connections between the “weaker” eye and brain can lead to permanent vision loss if not aggressively treated in childhood. 👁️Keratoconus: The cornea thins and bulges forward like a cone leading to permanent vision loss in young people. Worsening nearsightedness and astigmatism year after year may be the only signs of early disease (Check out my reel on how rubbing your eyes can cause keratoconus). 👁️Macular Degeneration: Age-related changes in the central retina slowly deteriorates your vision over time. Regular dilated eye exams and specially formulated vitamins can help prevent severe vision loss. 👁️Diabetic Retinopathy: High blood sugar can damage the delicate blood vessels in the back of your eyes without you realizing it. That’s why all patients with diabetes need routine dilated eye exams. 👁️Retinal Tear: A sudden onset of floaters or flashing lights could signal a retinal tear, but sometimes there’s no warning.
