Safe cold medications while breastfeeding?! 🤧

𝐏𝐚𝐢𝐧 & 𝐅𝐞𝐯𝐞𝐫:

✅Ibuprofen (L1): Treats pain, fevers, AND inflammation. The GOAT. 🐐

✅Acetaminophen (L1): Treats pain and fever. No anti-inflammatory properties.

𝐍𝐚𝐬𝐚𝐥 𝐂𝐨𝐧𝐠𝐞𝐬𝐭𝐢𝐨𝐧:

✅Nasal Saline Irrigation (L1): Used as needed helps thin mucus and break up congestion

✅Cool Mist Humidifier (L1): A staple for our kids - why not use for you too to break up mucus and decrease inflammation of the nasal passage?!

✅Intranasal Steroids (fluticasone L3): Aids in decreasing the inflammation in the nasal passages

✅2nd Generation Antihistamines (loratadine L1; cetirizine & fexofenidine L2) I recommend leaning away from diphenhydramine (1st gen antihistamine) despite its L2 category, as its more sedating and anticholinergic effects has more potential to make baby drowsy and dry up milk as opposed to the 2nd generation antihistamines.

🚫Pseudoephedrine (L3): Decongestant - AVOID! Studies have shown a single dose of 60 mg of pseudoephedrine can decrease 24 hour milk production by 24% (Aljazaf, et al.)

𝐂𝐨𝐮𝐠𝐡:

✅Pectin-based cough drops: Pectin is not absorbed in the GI tract and a better alternative than benzocaine (L2) and menthol (L3)

✅Guaifenesin (L2): Used for wet cough to break up mucus and cough junk OUT

✅Dextromethorphan (L3): Used for dry cough to suppress the coughing reflex

💡Tip: Don’t take guaifenesin and dextromethorphan together or the combo formulations of both - one is surpressing the cough (ideally for sleep), while the other makes it more productive to cough and clear secretions out (ideally for day). Remember coughing is a physiologic protective mechanism.

💡Generalized Breastfeeding Medication Tip:
💊Use immediate release instead of extended release to minimize exposure to baby
💊Opt for single medication products instead of combination cold medications to target specific symptoms, again limiting exposure to baby

💛Antenatal Colostrum Harvesting 💛

What are the benefits of colostrum harvesting?

✨ Mom separated from baby at birth

✨ Newborn hypoglycemia

✨ Immunity

✨ Constipation

✨ Practicing the skill of hand expression

How to harvest colostrum at 37+ weeks pregnant with OB approval:

1️⃣ Breast massage to stimulate oxytocin and allow for a more effective collection.
Tip: Sometimes taking a warm shower can help prior to collection!

2️⃣ Hand express by making a “c-shape” closer to the edge of your areola or where comfortable, pull back on the breast without letting your hands slip down to the nipple, and compress. Express each side for about 5 minutes until you get a good rhythm going.

Reminder: This is a skill that requires practice and patience - it is NOT about volume! 💦

Why hand express instead of using a pump? Colostrum is a very thick, sticky substance. Often times when mothers try to use a pump to remove colostrum, the majority remains in the pump flanges or valves. Hand expression is also a lot gentler than the suction of a pump!

$🥩 We’ve got BEEF with PPIs! And what better day to talk about it than World Osteoporosis Day?! 🦴 Three pharmacists: ➡️ @ketaminerx ➡️ @the.milk.pharmacist ➡️ @thecaregiverpharmacist teamed up to share three different perspectives on proton pump inhibitors (PPIs)! PPIs absolutely have their place in therapy, but long-term use isn’t always necessary. And with prolonged use potentially associated with increased fracture risk, it’s worth taking a closer look at WHY they’re still being prescribed. From the ICU to geriatrics to pregnancy and lactation, we’re breaking down our biggest concerns with this medication class. 💊 The takeaway? We’re not anti-PPI. We’re pro-appropriate prescribing, regular reassessment, and deprescribing when it makes sense! Swipe through to hear all three perspectives! 🥩 by @the.milk.pharmacist$

A recent preprint on leftover breast milk has understandably gotten a lot of attention, especially from parents who have had to throw away milk their baby didn’t finish.

The findings ARE interesting.

Researchers found limited bacterial growth in leftover milk during the time periods they studied. That could eventually help inform whether current recommendations are more conservative than necessary.

But there are some important caveats: this was a small study in Germany of 44 healthy, full term infants, and it measured bacterial growth. It has also not yet been peer reviewed.

So while this research is worth paying attention to, it does not establish that leftover breast milk is safe to store and feed for longer periods, and it has not replaced current recommendations.

There’s nothing wrong with questioning guidelines. In fact, that’s how science progresses.

The evidence may eventually support changing recommendations. We just aren’t there yet. 🖤 by @the.milk.pharmacist

🔥𝗲𝘂𝗳𝘆 𝗦𝟮 𝗣𝗿𝗼🔥

As a CLC, I’m really selective when it comes to utilizing and recommending a pump and the eufy S2 Pro checks A LOT of those boxes:

⚪️360° clear, see-through cups: Easier to ensure nipple alignment and milk output

🌙 Built-in night light: Because trying to line up your nipple during a MOTN pump in the dark? 😵‍💫 This makes it SO much easier.

🔥 Heat + vibration: Designed to help stimulate the breast and support milk flow

📏 Multiple flange inserts: Because flange fit matters!

⏱️ eufy reports 35% more output in 30% less time!

✋🏻Wearable + hands-free: Whether your a SAHM mom trying to chase and cook for multiple kids or a working mom rushing to school pick up - this ensures we are still removing milk during crazy schedules!
