Evaluators are often familiar with the classic signs of Autism seen in boys—like obvious repetitive behaviors or strong fixations on specific topics. But in girls, Autism can look very different, making it easy to miss. Here are some of the most commonly overlooked signs that women report to me every week.

💬 **Intense Interests (But Socially Acceptable Ones):** Instead of trains or numbers, their deep passions may revolve around books, animals, or celebrities—topics that seem more “typical” but are pursued with extraordinary intensity.

💗 **People-Pleasing & Sensitivity:** They may be highly empathetic, deeply affected by emotions, and work hard to fit in—even at their own expense.

😣 **Social Confusion (That Looks Like Anxiety):** Many autistic girls experience friendship difficulties but internalize them as anxiety or self-doubt rather than outward social withdrawal.

🌀 **Sensory Sensitivities (Often Subtle):** They may dislike certain clothing textures, bright lights, or loud noises but endure discomfort rather than vocalizing it.

⚖️ Strong Sense of Justice: Many autistic girls have a deep commitment to fairness and may get upset when they see rules being broken or people being treated unfairly.

🎭 Chronic Overthinking & Social Anxiety: They may constantly analyze conversations, worry about saying the “wrong” thing, and struggle with social expectations.

💤 Routine & Predictability: While they may not show obvious repetitive behaviors, they often rely on routines, rituals, or strict personal rules to feel safe and in control.

Recognizing these signs can change lives. If this sounds familiar—whether for yourself or someone you love—know I’m here to help. Reach out for an evaluation (link in bio) or follow to stay informed. Also like or share to get the word out 💙

1. You create scripts in anticipation of social interactions.

2. You feel lonely even when you’re with others.

3. You have friends but don’t feel deeply connected to them.

4. You are focused on verbal and nonverbal responses, trying to make sure you get it “right”.

5. You mimic the actions or statements of others.

It’s important to unlearn all of the things you internalized as “acceptable”behavior. Follow for more tips on thriving vs Surviving after an adult Autism diagnosis.

You went through the process, shared your experiences, and still didn’t receive an autism diagnosis—why? Here are some common reasons this happens:

✏️Masking & Coping Strategies – Years of adapting can make traits less obvious to professionals.

✏️Outdated Assessment Tools – Many tests were designed for children and don’t capture adult experiences.

✏️Stereotypes & Bias – If you don’t fit the traditional image of autism, your traits may be overlooked.

✏️Misdiagnosis – Anxiety, ADHD, or depression might be diagnosed instead of autism.

✏️ Lack of Childhood Records – Many evaluations require early-life evidence. Talking to your parents or bringing them to the evaluation may be helpful if you’re comfortable with that.

✏️ No preparation - Not writing down signs or symptoms and preparing a detailed history.

✏️ Many signs and symptoms of Autism overlap with other possible diagnoses that may better explain your presentation!

What was your experience with an evaluation? Did you face any barriers? Follow for upcoming posts on how to change this and share with others who might benefit from this information.

Many autistic girls and women go years, sometimes decades, without a diagnosis but certain life transitions expose the difficulties they’ve been working so hard to manage.

Here are 8 common turning points:

1. Puberty: Social hierarchies become more complicated, friendships shift, sensory sensitivities intensify, and the pressure to fit in increases.

2. When they go to college: Roommate conflicts, shared living spaces, sensory overload, unstructured schedules, and navigating new social expectations can expose previously hidden difficulties.

3. Entering romantic relationships: Navigating dating, intimacy, unspoken expectations, boundaries, and the emotional complexity of relationships can expose longstanding social communication differences.

4. When they become mothers: Sleep deprivation, constant sensory demands, unpredictable routines, and the mental load of parenting can trigger significant autistic burnout.

5. When workplace responsibilities increase: Promotions, leadership roles, office politics, networking, and managing competing demands can become increasingly difficult.

6. During perimenopause: Hormonal changes, sleep disruption, and reduced capacity to mask or compensate can make autistic traits much more noticeable.

7. After a major life disruption: Divorce, bereavement, illness, or caregiving responsibilities can overwhelm the routines and coping strategies that previously kept everything together.

8. Retirement: Losing the structure, routines, and predictable expectations of work can make executive functioning challenges, social isolation, and difficulty creating new routines much more apparent.

These women didn’t suddenly become autistic. They reached a point where the demands of life exceeded their ability to compensate for an undiagnosed neurodevelopmental difference.

So many autistic people tell me that CBT therapy in the traditional sense has not helped them! Not only that they walked away feeling like they were the problem! It is very important that anxiety be viewed through an autistic lens for therapy to work. This guide provides strategies you can use on your own or with your therapist.

8 humiliation rituals autistic people are expected to accept as NORMAL.

1. Being expected to make small talk with strangers, coworkers, cashiers, and acquaintances.

2. Having to laugh at jokes that aren’t funny because someone expects you to demonstrate that you’re socially engaged.

3. Having to use a customer service voice, manufacture a cheerful tone, and sound pleasantly interested when interacting with people.

4. Having to pretend you’re interested in conversations about topics you find completely uninteresting.

5. Being pressured to hug, kiss, or physically greet relatives you barely know.

6. Having to manage everyone else’s feelings when you decline an invitation, leave early, don’t answer every family text, or choose to spend a holiday differently.

7. Being expected to attend workplace happy hours, holiday parties, and after-work dinners to prove you’re a team player.

8. Having to sit through office birthday celebrations, sing-alongs, group photos, and forced celebration.

Think about how much of everyday social life requires autistic people to continually demonstrate that they’re sufficiently pleasant, interested, enthusiastic, agreeable, and available to other people.

And then consider how rarely anyone questions why these performances are mandatory in the first place.

I explore the psychology of these experiences and share practical self-healing strategies at The Autism Doctor on Substack. Come join the conversation away from the trolls! Comment Stack and I’ll send the link!

I can’t tell you how many autistic adults come into my practice carrying deep shame because Cognitive Behavioral or some other type of therapy didn’t work for them. They’ve spent years trying to “thought-challenge” their way out of panic, only to feel more exhausted, broken, or gaslit in the process.

Here is what more traditional mental health advice often misses: when your nervous system is completely dysregulated from sensory overload, masking, or constant unpredictability, your logical brain is essentially offline. Asking someone to reframe their cognitive distortions in that state is like trying to fix a house fire by rewriting the home manual.

Real relief for autism-distinct anxiety doesn’t start in your thoughts, it starts in your body and your environment.

If this cycle sounds familiar, today’s article breaks down autism distinct anxiety, why traditional therapy misses the mark, and lays out what actually works.

Some autistic adults describe a desire to disappear, become unreachable, escape consciousness, or simply no longer have to participate in the demands of being alive.

These experiences can be mistaken for suicidal ideation, even when there is no desire, intention, or plan to end one’s life.

Here are 7 experiences worth understanding:

1. Wishing you could disappear. Wanting to vanish from everyone’s expectations, demands, and awareness without actually wanting to die.

2. Wanting to stop being perceived. Feeling so exhausted by social interaction, masking, and other people’s expectations that you wish nobody could see, hear, or approach you.

3. Wishing you could cease to exist temporarily. Wanting a complete break from consciousness and experience without wanting to cause your own death.

4. Wishing you had never been born. Experiencing profound existential distress about the circumstances of your existence without necessarily wanting to end your life now.

5. Fantasizing about living alone in the woods. Imagining a life without social expectations, constant communication, sensory overload, or interpersonal demands.

6. Feeling disconnected from the future. Having difficulty imagining future milestones, making long-term plans, or relating to conventional life trajectories.

7. Wishing you could sleep indefinitely. Wanting an extended escape from sensory input, cognitive demands, emotional processing, and the relentless experience of being awake.

Autistic distress can be profound, and language about wanting to disappear or stop existing deserves to be taken seriously. However, it is important not to automatically interpret every expression of existential exhaustion as a desire to die.

Equally important: non-existence ideation does not rule out suicidal ideation. The two can coexist.

Autistic adults can be dangerously overwhelmed without actually feeling overwhelmed.

You may think you’re fine while your capacity is unnoticeably disappearing, until suddenly you can’t work, socialize, think, communicate, or tolerate another demand.

Then you wonder how you went from fine to completely unable to function!

The answer may be hiding in the interoception and alexithymia trap. Your body can be under enormous stress while your conscious awareness of that stress lags behind. That means “listen to your body” isn’t always enough. You need to know what your personal warning signs look like before the crash.

My new articles, “The Silent Body” & “Building an External Autistic Nervous System,” gives you a practical system for doing exactly that: identifying your pre-burnout signals, tracking your capacity, mapping your environmental load, and knowing when to intervene before your body forces you to stop.

If you’ve ever been shocked by how suddenly you crashed, this article will change the way you understand the warning signs you’ve been missing and help you to catch the crash long before it happens in 5 easy steps.

Visual stimming isn’t always staring at something or watching the same video over and over. Sometimes it’s the little things your eyes are drawn to because they provide sensory input, stimulation, or regulation.

Here are 7 examples:

1. Staring at spinning objects, like ceiling fans, wheels, or washing machines.

2. Looking at repetitive patterns, like tiles, wallpaper, stripes, or geometric designs.

3. Wanting everything to be a specific color. Decorating with, wearing, or buying things in your favorite color to degree that is way beyond others.

4. Looking at things from the corner of your eye rather than directly at them.

5. Staring at flickering lights, reflections, or changing colors.

6. Watching repetitive movements, like windshield wipers or rotating machinery for long periods.

7. Moving your fingers in front of your eyes and watching the changing shapes, shadows, or patterns.

You may have spent years thinking you were easily distracted, daydreaming, or simply fascinated by certain visual things. Some of these behaviors may actually be visual stimming: a way of seeking sensory input or regulating your nervous system.

I write about the sensory, emotional, and neurological experiences of autistic adults at The Autism Doctor on Substack. You’ll discover why your brain responds the way it does, recognize patterns you may have misunderstood for years, and learn practical strategies for making everyday life easier.
