The first dose didn't do much. A few weeks later, some symptoms eased but others didn't. Now you're three months in, wondering if this is even working or if you picked the wrong provider.

Most people don't start their metabolic optimization with sleep. They start with the supplement, the injection, or the peptide. The thing that feels fastest. The thing that feels most like progress.

And then they're frustrated when results stall. When energy doesn't improve the way they expected. When body composition barely moves despite real medical intervention.

You’ve taken the antibiotics and followed every bit of advice. It keeps coming back.

If you’re in your 40s or 50s, declining estrogen may be part of the picture. As estrogen drops, vaginal and urinary tissues can become thinner and more vulnerable, which can make UTIs more likely. This is called Genitourinary Syndrome of Menopause (GSM), and it’s treatable.

Your annual bloodwork probably includes fasting glucose and A1c. And if those numbers come back in range, you're told your metabolism is fine.

But glucose is one of the last markers to move. It stays in range while your body compensates behind the scenes, producing more and more insulin to keep blood sugar steady. By the time glucose rises, the dysfunction has been developing for years.

Fasting insulin tells a different story. It reveals how hard your body is working to maintain that "normal" glucose reading. And it can flag insulin resistance at its earliest, most reversible stage, long before glucose or A1c ever shifts.

The problem is, most standard metabolic panels don't include it. So patients walk away with a clean report while the earliest signs of dysfunction go undetected.

This matters because insulin resistance doesn't just lead to diabetes. It drives weight gain, inflammation, hormonal imbalance, cardiovascular risk, and fatigue. And the earlier it's identified, the more options you have to reverse it through nutrition, movement, and targeted support.

If your bloodwork has only ever included glucose and A1c, you may be missing the most informative marker on the panel.

A 2025 survey found that nearly 40% of perimenopausal women received the wrong diagnosis. The most common one? Anxiety or depression.

65% said their concerns weren't fully addressed. 26% said their provider didn't take them seriously.

And for most, the next step was a prescription for an antidepressant medication.

These medications typically work by increasing available serotonin. But in perimenopause, the issue often isn't how your brain processes serotonin. It's that your body is producing less of it. Estrogen directly supports serotonin production. When estrogen starts to decline, serotonin output drops, and mood, sleep, focus, and emotional steadiness change with it.

An antidepressant can ease the symptoms. But it doesn't address why serotonin dropped in the first place.

A complete evaluation starts with mapping estrogen, progesterone, cortisol, and thyroid, because when one drops, the others move with it. From there, a targeted plan addresses the hormonal root, not just the emotional symptoms.

If you've been on an antidepressant and you still don't feel like it addressed everything, the missing piece may be hormonal.
