OSTEOARTHRITIS
Osteoarthritis (OA) diseases are a result of both mechanical and biologic events
•It involves destabilization of the normal coupling of degradation and synthesis of articular cartilage, chondrocytes, and extracellular matrix, and subchondral bone
•It particularly affects individuals over the age of 65years and is the major cause of hip and knee replacements in developed countries
•OA is cause by multiple factors including joint integrity, genetic predisposition, local inflammation, mechanical forces and cellular and biochemical processes.
Also Increasing age, Gender, Genetic predisposition (Type 2 collagen gene defects), Obesity (most important modifiable risk factor), Previous injury (either due to trauma, sport or occupation), Previous disease such as rheumatoid arthritis or gout, Systemic disorders such as acromegaly, Neuropathic joint disease such as a Charcot joint and Congenital abnormalities of the hip
*Osteoarthritis (OA ) falls into 2 major classes:
•Primary (Idiopathic)
• Localized OA, involving one or two sites
• Generalized OA, affecting three or more sites
•Secondary OA (associated with a known cause)
•Rheumatoid or another inflammatory arthritis
• Trauma
•Congenital factors etc.
TREATMENT GOALS
•Educate the patient, caregivers, and relatives
•Relieve pain and stiffness
•Maintain or improve joint mobility
•Limit functional impairment
•Maintain or improve quality of life
MANAGEMENT APPROACH
• Education, advice and access to information
• Strengthening exercises to improve muscle strength and aerobic fitness training

Urinary tract infection (UTI) is one of the most common complaints seen in general practice and accounts for about one-third of hospital-acquired infections and commonest reasons for prescribing
antibiotics.
• Escherichia coli is the most frequent pathogen, accounting for more than three-quarters of community-acquired urinary tract infections. Symptoms are variable; many UTIs are asymptomatic and some present atypically, particularly in children and the elderly. Asymptomatic UTI should be treated in children and pregnant women.
E. coli is becoming increasingly resistant to amoxicillin,cephalosporins, trimethoprim and the quinolones. A 3-day treatment course is usually sufficient in uncomplicated
lower UTI in women. Longer, 7–14-day courses are recommended for men, children and pregnant women. While it is not always necessary to send urine samples for laboratory analysis unless empirical treatment fails, they provide local epidemiology and antibiotic resistance data.
• Antibiotic prophylaxis may be beneficial in women with recurrent UTIs and in children with structural or functional abnormalities. by @_likhem
