Case Report
A 50-year-old patient was referred for endodontic management following the separation of an endodontic instrument in tooth #43 by the general practitioner.
Preliminary Radiographic Findings:
The radiograph revealed a long nickel-titanium (NiTi) file occupying the entire canal length. Instrument separation occurred at the cervical third, likely due to cyclic fatigue. Notably, the anatomical configuration was atypical, with the presence of two distinct roots.
Anatomical Considerations (Slide 5):
The occurrence of two-rooted mandibular canines is rare, with a reported prevalence ranging from 0% to 12%, depending on the studied population.
Management of the Separated Instrument (Slide 6):
The management strategy for a separated instrument must consider multiple factors, including those related to the instrument itself, using the Size-Length-Location (SLL) classification. Based on this assessment, a decision is made to either bypass or retrieve the fragment. In this case, removal of the separated instrument was deemed the most appropriate course of action.
Instrument Retrieval Technique:
Various techniques are available for instrument retrieval, and the choice largely depends on the clinician’s experience and the available armamentarium. In this case, a combined approach utilizing ultrasonics and the braiding technique was employed:
• The access cavity was carefully modified to improve visualization and straight-line access.
• An ED6 ultrasonic tip (Woodpecker) was used to loosen the separated fragment within the canal.
• Two H-files were positioned around the fragment in opposing directions, engaging it firmly.
• The files were braided together and withdrawn, successfully retrieving the separated instrument in a single piece (Slide 3).
Subsequent Canal Preparation:
After achieving working length determination in both canals with a size #10 K-file, canal shaping was performed using a WaveOne Gold Primary file (31 mm, Dentsply Sirona). Irrigation was carried out with 10 mL of 5.5% NaOCl activated ultrasonically, followed by a final rinse with liquid EDTA.
Obturation was completed using AH Plus sealer (Dentsply Sirona) and the WVC technique. by @ali_houseini
