High-performers don’t burn out because of a lack of discipline. Often, the engine is just running on empty.

In my practice, the high-achievers sitting across from me—executives, athletes, and artists—frequently come in battling a sudden wall of brain fog, plummeting motivation, and shattered focus.

Before chasing complex fixes, I always check one major hidden culprit: ferritin.

Your brain needs stored iron to synthesize the dopamine and norepinephrine required for focus and drive. When ferritin tanks—even without full-blown anemia—the cognitive toll mimics adult ADHD: scattered attention, severe mental fatigue, and white-knuckling through the day.

As a physician, the first step in addressing sleep issues is always to rule out underlying pathology — conditions such as sleep apnea, restless leg syndrome, thyroid dysfunction, chronic pain, depression/anxiety disorders. 

But once those are evaluated, what often remains is the deeper challenge: helping the mind truly rest. I had the privilege of moderating a panel on sleep — not just as a symptom, but as a reflection of the nervous system’s state.

Too often, patients with insomnia are prescribed medication. Yet time and again, every meta-analysis and updated guideline from the American Academy of Sleep Medicine affirms that Cognitive Behavioral Therapy for Insomnia (CBT-I) remains the first-line treatment. When I was in residency, I would “prescribe” CBT-I, but the behavioral health team often didn’t know what to do with it. This is because despite being so strongly recommended, CBT-I remains a niche and highly specialized therapy. (Shoutout to @empowersleep @sahilchopra.md who are helping make it more accessible.)

At its core, CBT-I reminds us that sleep begins in the nervous system. The work is in helping the mind feel safe enough to let go; in cultivating a physiology that allows rest to arise, not be forced.

A new pilot study found that people with Alzheimer’s who took 20g of creatine daily for 8 weeks had:

🧠Improvements in working memory, attention, and reading comprehension
