A hysterectomy doesn’t always mean the hormone conversation is simple.

For women with a history of endometriosis, European guidelines recommend avoiding automatically using estrogen-only hormone therapy because of concerns about possible malignant transformation.

This doesn’t mean every woman needs progesterone. It means the reason for your hysterectomy matters and your hormone plan should be individualized.

My mother, Sandra, was the healthiest person I knew.

She walked every day for years. She ate well, stayed active, and took her health seriously long before it was a trend. She was on hormone replacement therapy and it was working.

Then her doctor told her to stop at 72. The research was too risky, she was told. So she stopped.

What followed was gradual and then devastating. Her bones weakened. Two total knee replacements. The daily walks she had done for years became impossible.

She moved to assisted living in Florida. She became embarrassed about what she could no longer do. She withdrew socially.

The woman who always cared about her health looked at me one day and said what's the point, who cares.

That was the moment I knew something had been taken from her.

We now know that estrogen is neuroprotective. It plays a direct role in brain health, memory, and cognitive function.

Stopping hormone replacement at 72 did not just affect her bones and her joints. I believe it accelerated her cognitive decline. The research increasingly supports that connection.

She developed dementia. She broke her hip. Six months later, just before her 81st birthday, she was gone. December 5, 2021. Failure to thrive. A phrase that does not come close to capturing what actually happened.

Sandra did not fail. The system failed her. A doctor following outdated guidelines took away the one thing that was keeping her strong, her bones, her brain, her independence.

Nobody told her what the consequences might be.

I built Arcara Access because I could not save my mother. But I can give other women what she lost. by @arcara_access

Noticing your cholesterol change during midlife? Your hormones may be part of the picture.

As estrogen declines during the menopause transition, changes in LDL and other cardiovascular risk markers can occur. That’s why looking beyond the standard cholesterol panel can give a more complete picture.

ApoB, Lp(a), triglycerides, and hs-CRP can provide additional insight into cardiovascular and metabolic health.

Your cholesterol numbers are worth understanding in context, not in isolation.

When your thinking feels different, it’s easy to blame stress, being busy, or simply getting older.

But cognitive changes can have multiple contributors. Hormonal fluctuations may be one piece of the picture, while sleep, medications, mood, thyroid health, stress, and other factors may also matter.

The goal isn’t to assume hormones are the answer. It’s to understand the pattern and what may be contributing to it so your care can be individualized to you.

Muscle is one of the best investments you can make as you age.

Strength training isn’t just about looking strong. It supports your metabolism, balance, mobility, cognitive health, and independence.

The goal isn’t simply to live longer. It’s to stay strong enough to keep doing the things you love, on your own terms.

Start where you are, challenge your muscles, and build from there. Your future self will thank you. 💪

Poor sleep. Anxiety. Fatigue. Brain fog. Changes in mood.

It’s easy to treat each one as a separate problem. But sometimes, looking at each symptom on its own can mean missing the bigger picture.

That doesn’t mean every symptom has the same cause. It means the pattern, timeline, health history, medications, lifestyle, and other contributing factors can matter when figuring out what’s actually going on.

Personalized care starts with asking more than, “What symptom do we need to treat?”

It asks, “Why is this happening, and what else should we consider?”

Changing progesterone during perimenopause may affect more than your cycle.

It can influence the brain’s calming pathways, sleep, and how you feel overall.

If your sleep has changed, your hormones may deserve a closer look.

One of the most confusing parts of perimenopause is that symptoms aren’t always consistent.

Hormone production can become more variable as ovulation becomes less predictable, so your symptoms may change from one cycle to the next.

You might feel better one month, then experience disrupted sleep, mood changes, brain fog, hot flashes, or period changes the next.

Because these symptoms can have other causes too, looking at the timing, pattern, cycle changes, and overall health history can provide a clearer picture.

Tracking your symptoms over several cycles can also give you useful information to discuss with your healthcare provider.

If UTIs keep coming back, antibiotics may treat each infection without addressing what’s making you more vulnerable to them.

During perimenopause and menopause, declining estrogen can thin vaginal and urinary tissues and change the vaginal environment, increasing the risk of recurrent UTIs.

For some women, low-dose vaginal estrogen may help address these tissue changes and reduce recurrent UTIs. It’s worth discussing with your provider to see if it’s right for you.

You deserve answers, not just another round of antibiotics.

Treating the Symptom Is One Thing. Understanding the Pattern Is Another.

A hot flash, poor sleep, mood changes, or brain fog can feel like separate problems.

But during perimenopause, symptoms can overlap, fluctuate, and change over time.

Looking at the bigger picture, including your symptoms, cycle changes, health history, and clinical evaluation, can help create a more personalized approach to care.

One of the biggest misconceptions about hormone therapy is that you need to be fully postmenopausal before considering treatment.

But perimenopause can last for years. During this transition, estrogen can fluctuate significantly and progesterone production may become less consistent.

That can mean symptoms show up long before your final period.

If you’re noticing changes in your sleep, mood, energy, cycles, hot flashes, or how you feel in your body, you don’t have to simply “wait it out” until menopause.

Your care should be based on your symptoms, medical history, and individual needs, not just whether you’ve had your last period.

For many women, the transition begins while they’re still having periods.

The changes can show up as irregular cycles, poor sleep, mood shifts, brain fog, hot flashes, or changes in libido.

And because hormones fluctuate during perimenopause, symptoms can come and go.

You don’t have to wait until your periods stop to start paying attention to what your body is telling you.
