I first saw this patient in 2024 when he was referred to me by @red.sez. Beyond the obvious orthodontic issues, he had splinted pfm crowns 12-22 (with a distal cantilever 13 Pontic) and overhanging margins, moderate localised anterior tooth wear, gingival disharmony, impacted 17 and 8s, and numerous endodontically treated teeth without cuspal coverage.

We elected to:
- extract the 8s and 32
- deconstruct 12-22 and provide non-splinted temps during orthodontic treatment
- intrude 11/21 and 33-43
- substitute the 14 into the 13 position
- indirect restorations of 13-23, 26 and composite restorations of 34-44 and 15
*crown lengthening of 11-23 would have been nice but not critical given the low smile line

Exceptional orthodontic treatment by the very talented @red.sez

Knowledge is power!

Especially when it comes to your smile.

One of the biggest misconceptions about veneers is that they always require significant removal of healthy tooth structure. The reality is that, whenever clinically appropriate, treatment should be planned to preserve as much of your natural tooth as possible.

That’s why choosing your clinician matters. Don’t be afraid to ask questions, understand the treatment being recommended, and learn why a particular approach is right for your smile. An experienced clinician will take the time to explain your options and prioritise your long-term oral health.

For this patient, Dr Mira carefully planned a conservative veneer treatment for the upper teeth, creating a beautiful, natural-looking smile while preserving as much healthy tooth structure as possible.

Every smile is different, and every treatment plan should be too.

If you’ve been thinking about veneers, we’d love to talk you through your options and help you decide what’s right for you.

Front 4 🦷 This patient was referred for management of a worn dentition. While there was generalised mild-moderate wear, the patient had no functional nor aesthetic concerns with the exception of the “short front teeth”. We took a conservative approach and decided to restore teeth 12-22 at the current VDO.

The wax-up was designed to facilitate limited tooth reduction at the buccal / palatal of teeth 11 and 21 including restoration of significant erosive lesions at the palatal aspect of teeth 11/21. Tooth 12 was prepared for a veneer and tooth 22 for a 3/4 crown given the existing proximal restorations.

Aesthetically, we elected to maintain a slight class 2 div 2 tooth position and no doubt @barrysmithceramics had fun with the layering for this one.

Just a reminder that aesthetics is subjective. And more importantly, mild-moderate tooth wear should be managed on a case by case basis. A “full mouth rehab”approach is rarely required and seldomly clinically justifiable.

A Step Toward a Natural Smile

Every case is approached with quiet attention to detail. Whether it’s a single tooth replacement or a full arch reconstruction, the goal remains the same — to restore what’s missing in a way that feels completely natural.

Careful planning, subtle shaping, and thoughtful material choices all come together to ensure the final result doesn’t stand out… it blends in.

Because true success in dentistry isn’t about making something look “done” — it’s about making it look like it was always there.

Mediocrity isn’t about comparison — it’s simply something we try to move beyond, with every case, every time.

100% credit to @drsara_nyuperio for the surgical help. You are the best and to Paul @barrysmithceramics for incredible lab support.

One of the tricker anterior rehabilitations I’ve done!
This patient refused orthodontic treatment despite my begging. Given the teeth were heavily restored and she is 60 years old, I felt we could still achieve a satisfactory outcome without orthodontic treatment.

I performed a diagnostic wax-up using the buccally positioned tooth 23 as my guide. This way the wax-up was predominantly additive. While the proximal and palatal surfaces required reduction given the extensive interproximal restoration, bulk at the buccal aspect of the anterior teeth could be largely maintained.

The final outcome is not perfect but a significant improvement and a natural outcome . @barrysmithceramics achieved a natural final result in keeping with the existing dentition and in particular, tooth 23!

Sometimes the challenge isn’t replacing a tooth — it’s working within the limitations around it.

This patient presented with a missing 22, soft tissue deficiency at the pontic site, slight gingival recession on 21, a canted midline, and canines positioned at different vertical levels. Orthodontic or surgical correction would have simplified the case, however the patient preferred to avoid further treatment in that direction.

The solution therefore relied on careful prosthetic planning.

Pontic site development allowed fabrication of a modified ovate pontic to support the soft tissue and create a more natural emergence profile. Preparations were kept minimal, guided by a diagnostic wax-up and silicone keys, while strategic finish line positioning helped close the diastemas, avoid black triangles and improve anterior tooth proportions.

Working within the biological and aesthetic limitations of the case to create balance and harmony.

Big thanks to Paul and Allan @barrysmithceramics for the beautiful ceramic work and craftsmanship. I LOVED the seatting jig for the resin bonded bridge 😍

✨ When Challenge Meets Reward ✨

Sometimes, the higher the difficulty of a case, the greater the reward 💫

This patient presented with a history of cleft lip and palate repair, missing lateral incisors with implants placed at a young age, and old crowns on her maxillary central incisors. Unsurprisingly, she was unhappy with the aesthetics and function 😔➡️😊

We began by optimising oral hygiene 🪥, followed by meticulous planning and replacement of her existing restorations with new zirconia crowns 🦷✨
Although this was a four-unit case, the real challenge was in the details — long preparations, preserving pulpal health ❤️, and achieving harmony between tooth position, aesthetics, and a compromised lip form 👄

Balancing biology, function, and beauty was key 🔑

The result? A confident, natural smile and a very happy patient 😁💖
At her review appointment, she shared some beautiful news — she is now expecting 👶✨

Proof that a smile really can change lives 🌈

Huge thanks to Paul and Allan @barrysmithceramics for the incredible ceramic work, understanding my vision and bringing it to life. You are truly an artist ❤️

Localized anterior wear managed using the Dahl technique.

Space was created in a controlled, biologically-friendly way. Digital planning allowed the end point to be visualised before any preparation, keeping the preparations conservative and enamel was preserved wherever possible.

I LOVE the pencil guidance - preparations dictated by the plan, not by the bur.

Grateful to Paul @barrysmithceramics for bringing the digital plan to life with such precise craftsmanship.

This patient presented with significant tooth wear from years of reflux and a nocturnal parafunctional habit. He wanted to improve the appearance of his teeth but was clear he still wanted them to look like REAL teeth with character and texture. Paul and Allan @barrysmithceramics shared our vision here 🦷🔥

✨ From Splinted to Individualised ✨

💡 Prosthodontics is often about solving puzzles — blending biology, mechanics, and artistry to create smiles that look and feel natural.

This case started with 4 splinted crowns that compromised aesthetics and biology. The patient had previously completed orthodontic treatment for congenitally missing lateral incisors. The canines had been moved mesially into the lateral space, creating a challenge in managing tooth proportions and space distribution.

🔹 The goal: Restore natural aesthetics while respecting biology.
🔹 The challenge: Converting canines into laterals and premolars into canines, while utilising the excess space available.

We used a combination approach:
✔️ Direct composite veneers on premolars "canines".
✔️ Indirect layered zirconia crowns to deliver natural translucency and strength. As usual, wonderful ceramic work by Paul @barrysmithceramics

Thanks @maoviee for your referral of this lovely young lady.
