Trans oseophageal echo basics explained :

How it works: A specialized probe with an ultrasound transducer is passed down the esophagus, which sits directly behind the left atrium. No lung or rib interference means significantly higher resolution, especially for posterior cardiac structures.

Key indications:
→ LA/LAA thrombus exclusion before cardioversion or ablation
→ Infective endocarditis — detecting vegetations TTE may miss
→ Prosthetic valve function and paravalvular leaks
→ Native valve pathology — MR mechanism, MV prolapse, endocarditis
→ Aortic dissection/intramural hematoma
→ Intraoperative guidance during valve repair or replacement
→ Congenital defects like ASD, especially pre-closure planning.

Resistant Hypertension expert consensus : part 2 of AHA consensus.

If BP ≥130/80mm Hg despite maximally tolerated doses of ACEi/ARBs + CCBs + THZ/ THZ like diuretics:

1) optimise the dose of above
2) add beta blocker next if compelling indications.
3) add alpha blockers like Prazosin if beta blockers aren’t tolerated.
4) add another diuretic if volume overload.
5) if nothing works : Add :
Aldosterone Synthase Inhibitors
Alpha Antagonists
Central Alpha Antagonists
Endothelin Receptor Antagonists
Potassium-Sparing Diuretics
Vasodilators.

Husband’s cry mourning wife’s death.

Background to the POEM :

A middle-aged woman was admitted to the ICU when I used to work at KEM, Mumbai with autoimmune encephalopathy.
Her only companion: a husband, while their two young children stayed home with grandparents.
He was her caregiver, her nurse, her anchor. He fed her, cleaned her, maintained her dignity. He slept on the corridor floors, alert at all hours, never once asking for pity, never once complaining.
Slowly, she regained strength — weaning off the ventilator, moving to the ward. But then, a sudden hospital-acquired infection consumed her, and despite all our efforts, she slipped away.
When I declared her passing, her husband froze. Every cell in his body seemed numb. His mourning was raw, wordless, infinite.

We are quite fortunate to experience life full of Love, cause once it slips, all of our materialistic achievements turn worthless.
