We’re excited to welcome our new provider, @collinsjake to Haven Health! 👋

Jake brings 7+ years of experience in adult primary care and LGBTQ+ health, caring for patients across everything from preventive visits to more complex ongoing conditions.

Before becoming a Physician Assistant, Jake built a foundation in Public Health and community health leadership. He holds a Master of Public Health focused on health behavior and health promotion and previously worked in HIV research and community health programs in Los Angeles.

He later earned his Master of Clinical Health Services from the University of Washington School of Medicine’s MEDEX Physician Assistant Program, where his graduate research explored the impact of PrEP on sexual risk behavior, reflecting a long-standing commitment to HIV prevention and LGBTQ+ health.

Jake’s care style is thoughtful and collaborative. He believes the best care starts with listening, understanding the full picture of someone’s life, and building a plan together.

Whether it’s watching you do your thing on stage at @wehopride, or holding your hand across the table as we cry together, only to be laughing 10 seconds later, I wouldn’t trade any of it.

Life can be really hard sometimes, but it feels just a little lighter knowing I have a friend who will be there no matter what.

I have a feeling there might have been some tags/messages that I missed in the debauchery that was Weho Pride weekend, so here’s a retrospective THANK YOU to everyone for the birthday wishes. I may be starting 36 with a few more grey beard hairs, but all in all I’m feeling very blessed 🥲

Feeling a little mushy today as I say goodbye to a workplace that’s been home for the past five years. It was the first place I worked as a provider where I truly felt like I was doing something meaningful for my community.

Even though it’s time for the next chapter, I feel incredibly grateful for the chance to grow both personally and professionally alongside some truly amazing people. I’ll always carry a lot of appreciation for the team and the experiences that shaped me here.
