For Jax Capistran, Front-End Instructor with Care Model Academy, Hispanic Heritage Month is a time to celebrate her Mexican-American identity, carry family traditions forward and honor the generations who shaped her.

From gathering for Nochebuena to teaching her children to honor loved ones on Día de los Muertos, Jax finds meaning in the traditions that connect her family across generations.

At CommUnityCare Health Centers, she brings that same sense of familismo, putting family and community first, to her work training team members and serving the neighborhoods where she grew up.

Highlights include:

🥈 Silver-level Advocacy Centers of Excellence (ACE) designation for CommUnityCare’s commitment to advocacy.
🌟 Emerging Leader recognition for Yajaira Saldivar-Solis, Director of Grants and Strategic Initiatives.
🔬 Research presentations by Rehab Abdelfattah, Graham Block and Rana Mohsen on tuberculosis identification and management in community clinics.
🤰 CarePartners program evaluation presented by Yui Ayutyanont, Director of Education and Research, earning a $1,000 Health Center Controlled Network honorarium for aligning with key network priorities. The research explored care coordination for high-risk pregnancies among underserved populations.

Hispanic Heritage Month Spotlight: Arlene Melendez 🇲🇽

For Arlene Melendez, Care Model Front-End Instructor at CommUnityCare Health Centers, Hispanic Heritage Month is a time to celebrate the traditions, values and connections that bring generations together.

From family celebrations like Día de los Muertos and Christmas Eve gatherings to the lessons of “trabaja duro” (work hard), Arlene’s Mexican-American heritage has shaped her approach to life and community.

Since joining CommUnityCare Health Centers in 2022, she’s found meaningful ways to give back to the community, reflecting a value her mother taught her: “Familia es todo, y la comunidad también.” Family is everything, and so is community.

As a member of the CommUnityCare Health Centers Community Advisory Panel (CAP), Sophie Ryland shares why reducing stigma and making sexual healthcare more accessible matters to her. Through her work with CAP and IDEALHealth, Sophie has seen how community feedback can help identify barriers to care and shape the patient experience.

Her message to the community: “Don’t be afraid to ask questions or seek guidance.” Being informed about sexual health should never be a source of shame.

As Sexual Health Awareness Month comes to a close, we’re continuing the conversation about access, education and care without judgment.
