The voice-based clinical journaling app that turns your stream of consciousness into structured, actionable insights.

Debrief just got massive updates.

Four new things:

Folders. Pull related debriefs into collections you name: your board-review set, the tough cases, your procedures. One debrief can live in as many folders as you want.

Your own rotations. Rename the presets or add your own, Cards, GI, Heme/Onc, Night Float, whatever you actually rotate through. Every debrief then files itself the way you think about your training.

A rebuilt Insights. It is now built around a Daily Debrief you can finish in one sitting, with your streak, your heatmap, and more on the way.

Full editing ability. Reword a case, sharpen the memory hook, fix the rotation, or correct a line the transcript misheard. Every field is yours to edit.

Hey, can you look that up? Every attending has the same move. By the end of the day you’ve got ten of those, and you get to maybe one.

That pile is where most of the learning quietly dies. Debrief catches it instead. You flag the question out loud while you’re still on the floor. C. diff after clindamycin, fidaxomicin or vanc, whatever got thrown at you on rounds.

Later it hands you a real answer, pulled from actual references with the sources linked so you can check the original yourself. Save it and it becomes a card you review in the app or push to Anki.

Catch the look-ups while they’re fresh, actually close them out, and the next time it comes up, you just know it.

You spend the whole day with real patients. Then you go home and grind practice questions about imaginary ones. That never made sense to me.

Third year is the most clinical learning you’ll ever do, and most of it has nowhere to land. I had the Anking deck for UWorld, but for my actual patients, all I had were scratch notes on random scraps of paper that meant nothing two weeks later.

So I built Debrief. At the end of the day you just talk. The patient who stuck with you, the case you almost got wrong, the thing you keep meaning to look up. It comes back as a structured entry: a summary, the cases, your look-ups answered against real references, and pearls you can send straight to Anki. It keeps what you learned, not who the patient was. by @debrief.app

Things I learned from my patients, part 1: she was hypercoagulable from two directions at once.
Sweet patient with ovarian cancer. The cancer alone makes you clot more. But it had also triggered a paraneoplastic nephrotic syndrome, so she was losing antithrombin III, one of the body’s natural blood thinners, in her urine. Two separate mechanisms stacking in one patient, which made her anticoagulation plan something you actually had to think through.

The honest answer: anyone learning medicine in the room, not just the book. Third-years to attendings, and everyone on rotations in between.

It looks different at every stage. As a third-year, it is a place to try and balance all the things you learn across different medical specialties. As a fourth-year, it is a year of real patient stories ready the day ERAS opens. As an intern, it is the lesson from 2am coming back next week, right before you’d forget. PA and NP students, nursing, dental, every specialty, same deal.

What stays the same is the problem. The best teaching of your day feels unforgettable while it’s happening, which is exactly why no one writes it down. Catch it while it’s warm and you keep it.

Capture medicine in your own voice with Debrief, now live on the App Store.

I built Debrief because I think the best teachers in medicine are our patients, and I wanted a better way to capture everything they teach us, just by talking.

Whether on your commute home, or between bites at lunch, you can turn your stream of consciousness into a structured summary, with cases, pearls, growth moments, and the things you meant to look up, which you can check against references right in the app. And it brings those lessons back as flashcards over time, so what you learn actually sticks; you can use those flashcards either within the app or export to Anki.

A debrief is just you, talking through your day out loud while it is still fresh. Here is how to actually do it.

Where? Use the dead time you already have. The walk to the parking deck. The drive home. Between bites at lunch. Folding laundry. Making dinner. You are already replaying the day in those minutes anyway.

How? Talk like you are telling a friend. No notes, no script. Start with the patient you can’t stop thinking about. Tell the story, not the chart.

Skip the name, but keep the one detail that ties you to them. The way they talked. The thing they said in triage. That is your memory hook, and it is not anyone’s private information, because no one else knows what it points to.

What’s worth capturing, every time: the case, the lesson, the call you almost got wrong, the question you need to look up, and the moment that got to you.

The only rule that matters: sooner beats polished. Do it the same day, while it is warm. Two minutes is plenty, and messy is fine.

You will forget most of the lectures. You will not forget the first patient who scared you. That is not a discipline problem, it is how memory works. Emotion is the brain’s save button. The part of your brain that handles fear and stakes flags the moments that move you and tells the rest of your memory to hold on. It is why the case sticks and the slide does not.

There is a catch. Even a vivid memory fades if you never go back to it. The case that gripped you today is blurry by the weekend. So say it out loud while it is still warm. Two minutes of talking on the walk to the car keeps the story, and the feeling, instead of letting both cool overnight.

Swipe through for the research, with sources. Then debrief your day. On iOS, link in bio. by @debrief.app

Here is what Debrief actually does, screen by screen.

You talk for two minutes on the way home, about the patient who stuck with you or the thing you meant to look up. No script. Whether it is thirty seconds or thirty minutes, it has you. Can’t talk? Pull up the scratchpad and type a line instead.

It comes back as a structured entry: summary, key insight, cases, and the questions you flagged. Every “I should look that up” turns into a real answer with sources, saved next to the case that raised it. It all lands in a library you can search by rotation. Then the pearls come back as flashcards, on the same spaced-repetition engine as Anki, so the medicine you saw on shift is the medicine you keep.
