Post contrast single slice CT neck demonstrate ill demarcated heterogenous asymmetric thickening of the epiglottis more prominence in the left side, the lesion is invading the pre and para glottic spaces, as well as thyroid cartilage and strap muscles.
No lymphadenopathy evident.#reviewradiology #updateradiology by @diagnosticradiography

T2 shine Through: If the lesion has very bright T2 signal, DWI will not be able to mask it totally, then it appears of bright signal in DWI & ADC map.
This phenomenon is somewhat like a curtain that does not completelyblock out strong light.
T2 shine through has no diagnostic value.

PEARLS:

To diagnose true restriction, a finding must be bright/ hyperintense on DWI and dark/ hypointense in ADC, if it is bright in both, it might be a false positive artifact known as "T2 shine- through"

1. Diffusion usually measures the movment of of extracellular water molecules.
2. Diffusion restriction (DWI) refers to the phenomenon where the water molecules are limited in their normal random movment within the tissue.
3. The ADC map is a calculated mathematical image that measures the actual rate of diffusion.
4. The radiologist look at two specific sequences together to confirm true restriction.
a. DWI
b. ADC (Apparent Diffusion Cofficient), the restricted area appears bright/ hyperintense in DWI and dark/ hypointense in ADC
