POSTS STORIES REELS TAGGED
I have gained over 30,000 new followers in just over 3 months, so I figured I would reintroduce myself. 🫶🏾

I share my beliefs, my passions, and what I’m working on in this post. As well as what kind of posts/content to expect.

I’m building Frontier Medicine, a new and unique field dedicated to solving sci-fi medical problems like human hibernation. Because I don’t think the revolution in medicine will come from making AI better at prior authorizations or improving efficiency of our current flawed system.

I believe a whole new paradigm of thinking and research is needed. Only by thinking about sci-fi problems can we work backwards and solve some of the greatest mysteries of human health and advance towards a future where humans are in hibernation pods on their way to Mars (it’s going to be my company that builds this- save this as proof)! 🚀

Comment “Frontier” if you would like to join the community and be a part of the future of medicine!

Is this case a medical miracle or an incredible understanding of physiology and pharmacology?

In 2018, rescuers in Thailand faced a medical problem almost without precedent:

How do you take children through a flooded cave system underwater, when panic itself could be fatal?

The solution was extraordinary: general anesthesia during the rescue.

Ketamine was chosen because it could be given intramuscularly, provided relatively stable hemodynamics, and better preserved spontaneous breathing and airway reflexes than many alternative anesthetics. Atropine helped control secretions. A positive-pressure full-face mask provided oxygen and helped protect the airway. And throughout the rescue, another threat was quietly developing: hypothermia.

This carousel breaks down the physiology, pharmacology, and clinical reasoning behind one of the most remarkable examples of austere medicine ever attempted.

Sometimes medicine isn’t about following the protocol.

It’s about understanding physiology deeply enough to build a new protocol where none exist.

This is Frontier Medicine Case 3.

🚨 YOU’RE THE DIVE PHYSICIAN. WHAT DO YOU DO?

A) Initiate an emergency ascent to the surface to restore oxygenation.

B) Immediately deploy a rescue diver, even before the support vessel is stabilized.

C) Send an underwater robot (ROV) to locate him and attempt to deliver emergency breathing gas.

D) Stabilize the vessel, locate him using the ROV, and coordinate rescue into a pressurized diving bell with resuscitation equipment ready.

At 90 metres underwater, every minute counts. Which option gives him the best chance of survival? 👇

I feel like I’m repeating this every couple months- please stop intubating DKA patients. The ventilator simply cannot match the massive minute ventilation from kussmaul breathing that these patients usually have, and causes an acute worsening of acidosis from resp acidosis due to additional CO2 rise.

Of course, if meets criteria, then intubate. But usually trial BiPAP before intubating.

I cover the physiology of these sick DKA patients in this post. Priority is always fixing the underlying metabolic acidosis with insulin gtt, IV fluids, and electrolytes, while supporting their resp system to manage the hyperventilation long enough until acidosis corrects.

An astronaut almost drowned in space!?!?! NASA called it one of the most serious mishaps in space walking history!

This is a true story of how Luca Parmitano’s calm demeanor and communication helped avoid a serious incident during EVA when his spacesuit helmet filled up with 1.5L of water from an unknown source.

This post provides the full explanation of this incident, as well as how NASA approached its investigation of this serious event they classified as “High Visibility Close Call”.

To watch the full 1 hour NASA video of this whole thing going down, comment “EVA” and I’ll send the link.

As I was finishing my ICU fellowship, I was really torn about what the right next move for my career was. I was offered an academic ICU job if I did a Masters in Quality Improvement. I said absolutely no. I’m not doing a masters just so I could get a job.

I was also just so exhausted from 2 years of Covid as an ICU fellow, and honestly just didn’t want to do medicine.

I wanted to take 6 months and travel the world. People told me it was a bad idea because I was missing out on attending level money and that I’d lose some of my ICU skills.

I was worried about the second part more than the first. But for me my mental health, wellbeing, and excitement for life was the top priority.

So I took a few months off. I went everywhere, checked off all my bucket list items (check previous posts if you’re curious what I did). 6 months turned into 8 months. It was the best time of my life!!

Then I came back and did some locum ICU work until I found the right type of setup I wanted.

Now I work 1-2 weeks ICU in a month, with the rest of the time completely free for me to travel, teach online and as a professor at a med school, and explore my passions like space medicine and building a startup.

This was literally my dream lifestyle!!

So my message to you:

Be bold and make your own path!! Don’t let anyone tell you what you can and cannot do. Creating your own path will be more fulfilling and exciting that following conventional paths that don’t really excite you!

And if you are stuck or want my perspective on things, comment or DM me! I try to respond to everyone.

Would you have risked 6 months off medicine after finishing 12 years of training while still in 6 figure debt??

Based on a true story. This is Frontier Medicine Case 4. We are continuing the series exploring the extremes of human physiology!

Harrison survives btw. But if you were the doctor on that boat, knowing the risks and physiology, what decision would you make to ensure he survives? Vote and comment below!

A very common condition after cardiac bypass to be aware of if the Post-Cardiac Injury Syndrome. And just because there is a pericardial effusion doesn’t mean you have to put a needle in it!

This post covers diagnosis and management of PCIS, Dressler’s syndrome, and approach to pericardial effusion without tamponade.

Save this and share with someone who loves the heart (probably not more than me though)!!

Late post from my time as a crew medical officer on an analog astronaut mission to the Mars Desert Research Station.

Would you go to Mars (if it was a one way trip)?

I know my answer 🧑🏾‍🚀🤠

Keep sound on loud for slide 3! You’ll want to hear this!!

Everyone calls Harrison Okene’s survival a miracle, but it’s really just physics and physiology.

This post breaks down the dramatic survival, rescue, physiology, and the story of Harrison Okene, who prayed the Psalm his wife had texted him the night before, while slowly running out of oxygen 30 metres underwater!

A calm and quiet code is a good code! That’s how you know if you ran it well.

Another tip is to think and summarize out loud as the code leader so the whole room knows your mental model and what we have ruled out/are working on.

If want a part 2 let me know and I can get into some of the higher level stuff.
