Shared with my patient’s consent.
✨

This morning, I received this beautiful message from one of my patients, who’s 69 years old and now back at the gym six weeks after his facial rejuvenation surgery (facelift, neck lift, upper and lower blepharoplasty, fat grafting)

He shared that people have been approaching him, noticing how refreshed and youthful he looks, without necessarily realizing he had surgery. This has been happening many times a week with many different people in different settings.

One person even told him he looked like himself, only 20 years younger. Another who had just met him thought he was actually in his early 40s (almost 30 years younger!) and couldn’t tell he had had surgery even after he told her he was 69 and had just had a facelift and comprehensive face and neck rejuvenation a few weeks earlier

For me, this is what natural facial rejuvenation is all about.

Not changing someone’s face. Not making them look pulled, distorted, or operated on. But restoring youthful contours while preserving the features, expressions, and identity that make them unmistakably themselves.

As a plastic surgeon, there are few things more rewarding than hearing that a patient feels this confident and happy in his or her own skin.

And the fact that he’s proudly sharing his experience with others makes it even more meaningful.

Thank you for trusting me with your face, and for allowing me to share this beautiful message!

Trusting someone with your face is a deeply personal decision.

This patient’s words capture so much of what I hope to offer every patient: thoughtful care, an appreciation for what makes you unique, and results that feel natural to you.

Thank you for this beautiful review and for the trust behind it. It means more than you know.

The greatest compliment is when a facelift feels less like a transformation, and more like a return to your truest self.

When expressions still feel like your own. When the people around you see you, not the surgery you’ve had. When even another Beverly Hills physician recognizes the work for its quality.

For me, a facelift is about far more than “lifting”. It’s about aesthetic judgment, achieving meaningful change while preserving individuality, and caring for someone personally through every step of the experience.

I’m deeply grateful for this patient’s words, and even more grateful for the trust behind them.

I dedicate myself to the face because it is central to how we see ourselves, our identity, our character, our confidence, and how we connect with the world.

It carries our unique expressions, and holds the features that make each of us who we are. When genetics, injury, illness, aging, or weight changes affect the face, the impact can really reach far beyond appearance. Is see that in patients every day, and it’s beautiful to experience how powerful the impact of facial plastic surgery can be on quality of life.

Caring for the face requires more than technical skill. It requires understanding what makes each patient unique, how their goals fit within the context of their personal journey, listening carefully to their concerns and aspirations, and respecting the balance between creating a desired change and preserving and celebrating individuality.

My dedication is rooted in the belief that the best result is not a different face, but a face that feels charming, natural, familiar, and truly your own.

Choosing a plastic surgeon, especially for something as personal as a facelift, is a decision built on trust. I’m incredibly grateful when a patient leaves our consultation feeling heard, understood, and confident about the path ahead.

Thank you for trusting me with your face and with your care. I never take that responsibility lightly, and I am honored to be able to give you results you can confidently be proud of and excited about.

Can a facelift be performed comfortably while you’re awake? Yes - but awake surgery is not for everyone.

In this video, I discuss the 3 anesthesia options I use for facelift and neck lift surgery:

• General anesthesia
• Twilight / IV sedation
• Awake surgery with local tumescent anesthesia

The right choice depends on the patient, the extent of surgery, medical considerations, and personal preference.

Awake facelift surgery is a very specific option that works great in the right context for the right patient - not necessarily the best option of everyone. It can be particularly appealing to patients who strongly prefer to avoid general anesthesia, as well as selected patients for whom certain medical or anesthetic considerations may make deeper anesthesia less desirable.

With careful local anesthesia and appropriate patient selection, the procedure can be performed while the patient remains awake and comfortable.

One of the things my awake facelift patients appreciate most is the experience afterward: the procedure finishes, they get up, walk out of the operating room, and begin their recovery without having undergone general anesthesia or IV sedation. For appropriately selected patients, this can make the immediate postoperative experience and recovery feel even more streamlined.

But not everyone is a good candidate - and not everyone wants or should be awake for a surgery. There is nothing wrong with choosing general anesthesia or twilight sedation, and it’s in fact the most common choice for most patients.

The goal is not to promote one type of anesthesia for everyone. It is to offer thoughtful options that take your whole health into account and personalize the entire surgical experience to the individual patient.

What happens to the beard during a male facelift?

It’s an important detail, and one that should be considered carefully when planning face and neck surgery in men.

During a facelift, the beard-bearing skin of the cheek is repositioned along with the deeper facial tissues. Depending on a patient’s anatomy, beard pattern, and the amount of repositioning required, this can bring some beard-bearing skin closer to the ear.

That’s one of the reasons male facelift planning is different.

Incision placement, vector of lift, skin redraping, sideburn position, and beard distribution all need to be considered to create a result that looks natural - not just younger.

In some patients, if present, a small amount of beard hair near the ear may ultimately be managed with simple, noninvasive options such as shaving, plucking, waxing, electrolysis, or laser hair removal. The exact approach is individualized not only to prioritize natural results and completely hidden scars, but also to optimize quality of life—keeping any long-term upkeep as simple, effortless, and carefree as possible, without creating complicated maintenance routines.

The goal of male facial rejuvenation is not to change what makes a man look like himself. It’s to restore a more youthful version of his own face while respecting masculine anatomy, hair and beard patterns, and identity.

So grateful for this thoughtful review and, above all, for the trust this wonderful patient placed in me throughout her revision rhinoplasty journey.

Revision rhinoplasty can be an especially challenging personal journey. Many patients come to me after a previous surgery carrying not only complex anatomy, scar tissue, or limited remaining cartilage, but also understandable concerns about undergoing another operation.

In this case, her thick nasal skin and the repercussions of her prior rhinoplasty required a highly individualized approach, including structural reconstruction with rib cartilage grafting.

But what means the most to me is also knowing that she felt heard, supported, and cared for - from our first consultation through surgery and recovery.

It is an incredible privilege to be entrusted with someone’s face, breathing, and confidence. I never take that responsibility or a patient’s trust for granted.

Thank you for allowing me and my team to be part of your journey, and thank you for taking the time to share such generous words. Deeply honored.

I’ve had issues with my nose for as long as I can remember. I had no formed cartilage in the tip, struggled with my breathing, and my nose would collapse when I smiled.
From the moment I found Dr. Ramly and spoke with his coordinator, Tara, I knew I was in the right hands. He listened to every concern I had and took the time to create a surgical plan that addressed both the functional and aesthetic issues while still keeping the results natural.
Just two weeks post-op, I already feel like every goal we discussed was achieved. My breathing is improved, I finally have the tip structure and definition I was missing, and my nose feels so much more balanced with the rest of my face.
To see results I already love this much at only two weeks post-op speaks volumes about Dr. Ramly’s surgical skill. Beyond that, both he and Tara have been incredibly kind, attentive, and supportive throughout the entire process.
I couldn’t be happier that I trusted Dr. Ramly with something that was such a big decision for me. I would recommend him without hesitation. 🤍 by @drelieramly

Do you always need your submandibular glands reduced during a deep neck lift?

Not necessarily.

Not every patient needs the same deep neck maneuvers.

A deep neck lift should be personalized to your anatomy and unique aging pattern to create a natural, elegant result while preserving your identity.

Depending on the patient, deep neck contouring may include:

• Subplatysmal fat contouring
• Digastric muscle contouring
• Submandibular gland reduction
• Platysmaplasty
• Additional jawline contouring maneuvers

The goal is not to “do everything”.
It’s to do exactly what your anatomy requires.

So wonderful to arrive at the office yesterday morning to this beautiful orchid and lovely message from one of my facial rejuvenation patients (name hidden for privacy).

A beautiful morning dose of positive energy that set the tone for my next patient’s OR: a facelift, deep neck lift, and upper blepharoplasty. A wonderful way to carry that positive energy forward into another patient’s journey.

I feel incredibly fortunate to be surrounded by such wonderful patients and such a wonderful team at Ramly Plastic Surgery.

Grateful for the trust my patients place in me, and grateful for the journey we share. 🤍 by @drelieramly
