Common misconceptions in cirrhosis & liver disease:
- They shouldn’t take Tylenol
- NSAIDS are safer than Tylenol in liver disease
- Tramadol is safer bc it’s not a “real opiate”
- Low platelets and/or elevated INR means they’re not at risk of clotting

The Rural Generalist | Part 6/6: And THAT is Rural Medicine 🩺❤️

I’ve spent this series talking about the clinical side of rural medicine. The responsibility, the autonomy, the limitations, and the unique challenges of being a physician in a small hospital.

But this last part? This is the part that’s hardest to explain.

Because in rural medicine, the people you care for aren’t always strangers. They’re your neighbors, your former teachers, your friends, your colleagues, and the families you’ve known your entire life.

Some days it’s hilarious. Some days it’s heartbreaking. And a lot of days, it’s both.

There’s a weight to caring for your own community that I don’t think you can fully understand until you’ve lived it. But there’s also an incredible privilege in being trusted to do it.

And despite all the challenges, this is the part I wouldn’t trade.

and THAT is rural medicine 🤠❤️

This case of chronic catatonic state chalked up to a TBI is one of the more extreme examples of polypharmacy, but I see various degrees of REAL, measurable harm caused by chronic, home med drug-drug interactions & side effects—All. The. Time.

So yes, I am going to review the risks, benefits, alternatives of every med on your list and it is VERY COMMON that polypharmacy actually IS causing or contributing to your symptoms.

A year ago I had the bright idea to refer to GABA receptors as “GPA” 👴 & NMDA “TDFW” receptors in the name of alcohol withdrawal education…

Though the kooky receptor education is the same, since the making of this video, I have transitioned to primarily phenobarb ICU withdrawal protocol & it works fantastically.
