Most people ask the WRONG questions before eyelid surgery, here are 5 things to talk through with your surgeon to assure you get the best result without complications.

1. Ask where your brows are sitting? A low brow can mechanically hood the eyelids and mimic excess lid skin. If the brow is low you may want to address this!!

2. Ask if ptosis may be playing a role? Unrecognized ptosis can show up more after surgery. Not a good thing!!

3. Ask how much skin is really safe to remove? Too much can risk incomplete closure. That can be a problem. For life!

4. Talk about conservative fat excision. Too much removed can mean hollowing. This can make you look older!!!

5. Assure no muscle excision. This protects blink and eyelid closure force. This is very important!!

Ask questions, get clear answers, protect your outcome both on appearance and function. Comment below. Have a great day. That’s a wrap!! taken in Beverly Hills, California by @drguymassry

Some cases are difficult. Others seem impossible. But these are the cases that make our work so special.

This young man suffered devastating trauma to his right eye and facial bones. Despite multiple surgeries, he was left with a damaged cornea, severe lower eyelid retraction, extensive scarring, and an inability to close his eye.

After seeing multiple surgeons, he was told there were few options left.

I told him this might be a “Hail Mary,” but we were going to try.

I asked my colleague @drraymonddouglas to join forces.

Dr. Douglas performed an orbital decompression to move the eye back, allowing me to reconstruct the eyelid with five procedures:

    1.    Lower eyelid retraction repair
    2.    Midface lift
    3.    Lower eyelid skin graft
    4.    Canthoplasty
    5.    Hard palate graft

Five procedures. Two surgeons. One goal: Give this young man another chance.

Look at the immediate result!

His eye can now close! And he may now be a candidate for a corneal transplant to potentially restore vision.

This is why we do what we do. Not every case is easy. Not every outcome is guaranteed. But sometimes, giving someone hope and a second chance is everything.

That’s what makes this job so special.

Saying no to surgery is part of doing right by you.

If the preoperative exam and evaluation show anatomy is too unpredictable, or your goals can’t be met, pushing ahead isn’t brave, it’s reckless.

A thoughtful no protects your eyes, your face, your function, and your trust. It’s not rejection, it’s honesty. When a surgeon says no, they’re putting your long-term well-being first.

Can lower eyelid color, texture, and fine wrinkles be fixed with surgery? Not necessarily—and trying to do too much surgically can lead to complications and disappointment.

So what are the better options? I break them down in the video:

✅ Lasers and other energy-based treatments
✅ PRP or PRF
✅ PDGF
✅ Exosomes
✅ Nanofat injections

The key: Know what lower eyelid surgery can—and cannot—safely address. Sometimes the best treatment isn’t more surgery.

Watch the video and let me know your thoughts below.

I love this video. Every once in a while, a patient explains better than I ever could how surgery changed their life.

This young man was born with congenital droopy eyelids (ptosis), but his eyelids were symmetric. After surgery elsewhere, he was left with significant asymmetry and an unnatural appearance.

It took two surgeries to correct—but just look at him now.

This is why these cases mean so much. It’s not just about eyelids. It’s about helping someone feel like themselves again.

Let me know your thoughts.

Do surgeons ever stop learning? NEVER.

I’m in Las Vegas at the AAFPRS Fall Scientific Meeting.

Why do I keep coming to meetings like this?

1️⃣ Learn.
2️⃣ Grow.
3️⃣ Improve.
4️⃣ Share ideas with great colleagues.

The recipe is simple: You don’t get better without making the effort to get better.

Do you agree?

Let’s put it to rest!!! Should I have my lower lid cosmetic surgery to get rid of eyebags from inside the lid - called transconjunctival blepharoplasty, or from outside the lid through the skin - called transcutaneous blepharoplasty??
Please listen. Let me know your vote.

😴 AFRAID OF GENERAL ANESTHESIA?

You’re not alone. Let’s bust 5 common myths that may be creating unnecessary fear:

1️⃣ “I won’t wake up.” Extremely unlikely in a young, healthy patient.

2️⃣ “What if I feel something but can’t tell anyone?” Very uncommon with modern anesthesia and monitoring.

3️⃣ “It will increase my risk of dementia.” No clear evidence of this in young, healthy patients.

4️⃣ “It kills brain cells.” No good evidence to support this claim in healthy adults.

5️⃣ “General anesthesia just isn’t safe.” Modern anesthesia is remarkably safe for appropriately selected patients.

🎥 Listen to the video for the facts.

Fear thrives on uncertainty. Knowledge is key.

Have a great day
