POSTS STORIES REELS

"I woke up and looked in the mirror and my face had fallen off."

That is a direct quote. So are these:

"My cheeks have inconveniently relocated to my jowls." "I caught myself in a shop window and suddenly realised I had become my mother." "I look in the mirror and I don't recognise myself. And not in a good way."

Thirty-five years in general practice, nearly twenty in cosmetic medicine, and thousands of women describing this to me. The words vary. One thing almost never does — they tell me it happened overnight.

And the standard reply is that it didn't. That she's tired, she's stretched, she stopped looking properly, and the shock is really about the three years she went without a proper look in the mirror.

I think that's wrong. Not unkind — wrong.

Skin is oestrogen-responsive tissue. Receptors right through it: on the cells that make collagen, on the pigment cells, the oil glands, the hair follicles. That puts it in the same category as bone. With one difference — it's the only one of them you can see.

And when oestrogen goes, two things reverse at the same moment. Less collagen made, and more of the enzyme that dismantles it. Not one problem. Two, running in opposite directions, compounding.

They are not describing a steeper slope. They are describing a switch being thrown. As far as I can tell, that is more or less what happens.

Swipe for the mechanism, the numbers, and the four things that actually help — none of which require anybody's permission. Full article at the link in bio. by @drjclinics

THE MOST MISUSED PHRASE IN COSMETIC MEDICINE

I spent fifteen years championing a phrase that turns out to describe nothing.

Collagen activation. Every injector says it. A fair slice of the public now says it too. I said it louder than most.

There is no biological process by that name. What the phrase actually covers is four genuinely different mechanisms — stretching collapsed cells, controlled injury, a foreign-body response, and threads trying to do two jobs at once — with different timelines, different reliability and different ways of failing.

And the biology underneath is stranger than the marketing. We were taught that skin sags because fibroblasts get old and lazy. Fifteen years of work out of the University of Michigan says very nearly the opposite: the cells respond to what they're anchored to. Fragment the scaffold and the cell loses its grip, collapses, and starts making more of the enzyme that dismantles collagen.

The cells are not broken. Their environment is.

Which changes the question from how do we wake up a lazy cell to how do we give a collapsed one something to hold onto.

Swipe for the whole argument. Full article at the link in bio — including the part about threads that I learned from patients rather than papers. by @drjclinics
