I remember when the guidelines switched from annual Pap to every three years and everyone was freaking out. Now it seems we’ve gone a little bit too far in the other direction. Please don’t delay your cervical cancer screening. Metastatic cervical cancer is an absolute monster. Early detection is the only cure. by @drjenzelo

“Can I take hormone therapy, even though (insert perceived contraindication)?” is one of my most common questions — and I’m often happy to say YES, you can consider MHT. But it’s crucial to understand the broad strokes yourself before approaching your PCP, since many providers are still misinformed on this topic. Absolute contraindications (where risks truly outweigh benefits) include personal history of hormone-sensitive breast or endometrial cancer, undiagnosed vaginal bleeding, active/recent blood clot, known clotting disorder, active severe liver disease, or personal history of heart attack, stroke, or coronary artery disease. If these apply, MHT likely isn’t right for you — but non-hormonal options exist. Relative contraindications need a thoughtful conversation, not an automatic “no.” Migraine with aura, uncontrolled high triglycerides, gallbladder disease, or family history of clots often just mean adjusting dose, formulation, or route — like a patch or gel instead of a pill. Myth-buster: both the Canadian Menopause Society and The Menopause Society confirm well-controlled high blood pressure and diabetes are NOT contraindications. So many women are wrongly told no. 🙅‍♀️ On heart health: the American Heart Association is clear MHT shouldn’t be started to prevent a first or second cardiac event in someone with existing cardiovascular disease. But for healthy women under 60 or within 10 years of menopause, timing and route matter most — transdermal options carry a more favourable cardiovascular profile than oral estrogen.
