-A talon cusp is a rare developmental dental anomaly where an extra, cone-shaped cusp projects from the cingulum of an anterior tooth. -It frequently affects the palatal (lingual) surface of permanent maxillary central incisors. Named for its resemblance to an eagle's talon, it consists of normal enamel, dentin, and sometimes pulp tissue.

-Talon cusps create deep developmental grooves where they meet the surface of the normal tooth crown. These crevices act as "food traps" that are notoriously difficult to clean, making the tooth highly susceptible to caries.

-Undetected decay can then breach the enamel and dentin to reach the pulp.

Unfortunately in this case, the only way to see a pulpal communication was with a cbct.
