Hi, I’m Dr. Nixon Goyal 👋🏻

ERAS 2027 Nephrology Fellowship Applicant | MD Internal Medicine (India) | ECFMG Certified

I’m an Internal Medicine physician from India with a passion for Nephrology, Academic Medicine, and Medical Education.

My journey into Nephrology began during a clinical elective at Florida International University, Miami, and grew through my MD research on coagulation abnormalities in CKD patients undergoing hemodialysis.

As a Senior Resident, I’ve gained experience in CKD, dialysis, AKI, electrolyte and acid-base disorders, and complex inpatient care. I’m currently rotating in Transplant Nephrology at Virginia Commonwealth University (VCU), gaining valuable exposure to kidney transplantation and post-transplant care.

Beyond clinical medicine, I create evidence-based medical education content and mentor medical students and IMGs preparing for USMLE, ERAS, and the Match.

I’ve applied for the 2027 Nephrology Fellowship Match this year and look forward to connecting with nephrologists, fellows, researchers, and educators as I continue this journey.

We tell ourselves…
“Once this phase ends”
“Once things settle”
“Once I reach that next level”
Then I’ll finally be happy.

But every time we reached “there”…
the definition of “there” shifted again.

That’s when it hits you —
Happiness was never future-dated.
It was never tomorrow.

Happiness is a decision.
And the only time you can choose it… is NOW. 💛

With this post, I’m starting my new journey on Instagram, not just as a doctor, but as someone who’s still learning, growing, and trying to make a difference in how we approach studying and life in medicine.

The 7 years between Step 1 and Step 2 weren’t a gap — they were growth. They gave me perspective, patience, and purpose beyond textbooks. I learned that timelines don’t define your potential; your persistence does. And sometimes, taking a longer road helps you arrive more grounded, more prepared, and more grateful for the journey itself.

I’m Dr. Nixon Goyal, MD in Internal Medicine and ECFMG Certified (USA), having cleared USMLE Step 1 and Step 2 CK, and through every phase of this journey I’ve learned that success isn’t about being the topper — it’s about staying consistent, self-motivated, and genuine.

I’ve never been the typical bookworm — I’ve always loved tech, systems, and smarter ways to learn.
And honestly, that’s been my biggest strength. Now I’m merging Medicine × Technology to help future doctors study smart, stay calm, and live balanced.

Corticosteroids in Ards CORTICOSTEROIDS IN ARDS: WHAT DOES THE EVIDENCE SAY?

YES — CONDITIONALLY RECOMMENDED

Especially in moderate–severe ARDS when initiated early.

📌 KEY EVIDENCE

1. DEXA-ARDS
→ Early dexamethasone was associated with better clinical outcomes.

2. Meduri Trials
→ Corticosteroids improved inflammatory response and pulmonary physiology.

3. ARDSNet
→ Increased ventilator-free days, but no clear mortality benefit.
→ Potential harm when corticosteroids are initiated after 14 days.

🔑 TAKE-HOME MESSAGE

Early corticosteroid therapy may be considered in moderate–severe ARDS, whereas late initiation (>14 days) may be harmful.
