A rare late complication after MRM

A patient, 15 years after Modified Radical Mastoidectomy, was found to have cerumen emerging through a small opening in the retroauricular groove.

Surprisingly:
• No pain
• No active discharge
• No current complaints

🔎 Diagnosis:
Postauricular mastoid–cutaneous fistula

An epithelialized tract can create a persistent communication between the EAC/mastoid cavity and the postauricular skin.

⚠️ Clinical pearl:
A dry, asymptomatic fistula doesn't always mean a disease-free cavity.
Always evaluate for residual/recurrent cholesteatoma and underlying cavity pathology before attempting closure.

👅 TONGUE-TIE (ANKYLOGLOSSIA) — GRADING & MANAGEMENT

A short lingual frenulum may restrict tongue movement, but diagnosis is primarily functional—not purely anatomical.

🔹 GRADING
Commonly described using:
• Coryllos classification — anatomical classification based on frenulum attachment
• Hazelbaker Assessment Tool (HATLFF) — assesses both appearance and tongue function

🔹 MANAGEMENT
✅ No significant functional impairment → Observation / reassurance
✅ Breastfeeding difficulty → Lactation/feeding support first
✅ Persistent significant functional restriction → Frenotomy may be considered
✅ Selected older children/adults or more complex cases → Frenuloplasty
