Yaşla birlikte renk koyulur, dişler yıpranır, diziliş bozulur. Çapraşık dişler ise bunun üstüne gülüşü iyice dağınık gösterir. Bu vakada kompozit lamina ile hem form hem renk hem de diziliş yeniden kuruldu.

Çapraşıklığın olduğu bölgelerde dişin köşelerine kontrollü ve ölçülü bir şekilde şekil verilerek yeni bir hat oluşturuldu. Üzerine katman katman, elle işlenen kompozit ile dişin doğal ışık geçirgenliği, yüzey dokusu ve yaşa uygun ton yakalandı. Sonuçta abartısız, yüze ve karaktere uyan, hep böyleymiş gibi duran bir gülüş ortaya çıktı.

Removing a Trauma Splint and Composite – A Minimally Invasive Approach

Removing a titanium trauma splint is one thing. Removing the remaining composite without damaging the enamel is another challenge.

My approach combines careful mechanical removal, fluorescence-assisted visualization under the dental microscope, and dedicated composite polishing instruments.

Shape is more important than shade! 🦷✨

Finishing and polishing composite restorations doesn’t have to be complicated. My simplified protocol (which I’ve adopted from my dear friend and mentor @javiertapiaguadix ) uses just two rubber wheels, a couple of burs, and a single brush.

🔹 Refine the shape first. Define the outlines and line angles to create the right perception of tooth shape and size.

🔹 Pre-polish and polish. Diacomp Plus Twist (medium and fine grit) helps achieve smooth transitions and a natural gloss.

🔹 Recreate the texture. For younger teeth, carefully sculpt lobes, grooves, and perikymata. For mature teeth, soften the prominent areas to reproduce natural wear corresponding to the aging of the tooth.

🔹 Finish with gloss. A goat-hair brush and diamond polishing paste provide the final shine.

The secret? A simple, controlled protocol, attention to morphology, and the right finishing details. Because natural-looking restorations are about more than just shade—they’re about shape, texture, and gloss.

This moderate fluorosis case was treated with bleaching first, followed by a combination of microabrasion, resin infiltration and composite bonding.

Here’s the immediate result before the patient starts orthodontic treatment. In my experience, combining minimally invasive techniques is often what works best for these types of cases 🤍

INFILTRATION – THE POLISHING PROTOCOL 💚
Resin infiltration does not end with light curing.
The infiltrant should penetrate the porous lesion body—not remain as a superficial resin layer on the enamel. Careful removal of excess material and precise polishing are therefore essential.
In this case, I used EVE FLEXI-D polishing discs followed by DIACOMP plus to achieve a smooth, high-gloss surface with seamless optical integration and reduced susceptibility to staining and plaque retention. Because the long-term aesthetic and biological performance of the treatment depends not only on what happens inside the lesion, but also on the quality of the surface we leave behind.
Polishing is not only the finishing touch - it is an integral part of the infiltration protocol.
