Monopolar RF is a crowded category, and crowded categories get sold on price per treatment.

XTherma, made by Tentech, per the manufacturer's published specifications: monopolar radiofrequency at 6.78 MHz, up to 400 W, a pulse around 1,200 ms, cryogen cooling firing seven times inside each shot, three tip sizes. That is the mechanism, and it is where we stop.

The line the proposal leaves out is the cryogen. It is a consumable with a delivery schedule, and a missed refill cancels a booked day. Before any RF demo, ask for three things in writing: the refill cost per treatment, the delivery lead time, and what happens when a shipment is late.

What is the refill lead time on the consumable in your newest device?

A GLP-1 program looks like a prescription from the outside. Take one apart and it is an operating system, with seven parts that have to be written down somewhere.

Intake that leaves a record. A named prescriber of record. The pharmacy that fills it, on file. What the patient was told, in writing. Follow-up that is already on the calendar. A plan for the phase after the goal. And a sequence for who calls whom if supply changes.

Slide 7 is the one most programs skip. In the STEP 1 trial extension, participants who stopped semaglutide regained about two-thirds of their prior weight loss within a year (Wilding et al., 2022). The program has to be designed for what happens after the goal.

Save this, then check your program against it this week.

The practice that asks me what to add next is often full on Tuesdays and half empty on Thursdays.

That is a throughput problem. A new service line competes for the same rooms and the same provider hours you are already struggling to fill, and it inherits the same leak.

Two numbers from last quarter, before you add anything:

• The share of your open appointment hours that actually booked.
• The share of first-time patients who came back for a second visit.

If either one is soft, that is where the next dollar goes.

Exosomes are the most-asked-about item on any regenerative list right now. They are also the one I would not build a menu around yet.

There are no FDA-approved exosome products. That is the FDA's own current statement, and the agency has continued sending warning letters to companies selling them.

The operator question is what you would say about the exact product in your fridge, to a patient or to a state board investigator.

A regenerative line that has to survive that question should start with the part of it you can document.

Follow for the operator view.

Body contouring consultations are turning up in practices that never marketed body.

The published specification on this one, from bodyESTIQ (Onda Coolwaves): microwave energy at 2.45 GHz that essentially bypasses the skin, with approximately 80% absorbed in the adipose tissue. Two handpieces, one working at 6–8 mm and one at 16–18 mm, both with sapphire contact cooling settable as low as 5°C. Non-invasive, no downtime.

It targets fat, cellulite and skin laxity. The one result we will state is a temporary reduction in the appearance of cellulite.

Five questions the specification will not answer:

• How many longer treatment blocks your current room availability actually has, before you model any revenue.
• Whether a body protocol is sold as a series, and what that commits on a schedule you already sell.
• Which hours it fills — and whether those are hours your injector is not in the building.
• Who trains your second operator, and in which week.
• What consumables and service cost after year one.

Ask for all five in writing.

Save this before your next vendor call.

You will evaluate a pharmacy the way you evaluate a supplier. Price and turnaround.

It behaves like a dependency. It sits between your prescriber and your patient's next refill, and the questions that decide whether that works are the ones most operators only think to ask after the first time it fails.

Eight of them, one per slide.

Slide 3 is the one to use on the call. Ask for the name of the person your practice escalates to, and the path when they are out. A portal is not an answer.

Save the deck, then take it into your next partner conversation.

Your best injector can open a suite down the street for less than the cost of your last device.

AmSpa counted med spa locations going from 8,899 to 10,488 in a single year. I would bet a real share of that growth is somebody's former employee.

The common answer is pay. Pay is the one thing a competitor can match.

What survives a departure is enrollment — a patient in a program, with protocols they can name and a next appointment already booked. That patient keeps the appointment when the face changes. The one attached to a person leaves with the person.

If a provider gave notice on Friday, how many of your patients would still come in on Monday?

Five questions we would ask before adding a combined laser platform in 2026.

The published specification on this one, from Solunis Medical: diode at 755, 808, 940 and 1064 nm across a 4 cm² spot. SHR/OPT at 1 to 10 Hz behind a 480 to 690 nm filter set. Nd:YAG at 532, 1064 and 1320 nm, up to 1500 mJ. One 3000 W chassis, one 15.6 inch touch console, lamps rated 400,000 to 500,000 shots.

• What a replacement lamp costs, and how much of that 400,000 to 500,000 shot rating your projected volume uses in a year. Divide it out before you compare monthly payments.
• Whether two modalities can run at the same time, or whether one console means one queue.
• How many treatment rooms the platform frees, and how many it occupies.
• Who trains your second operator, and in which week.
• What service response time you are promised, and what it costs after year one.

Ask for all five in writing.

Save this before your next vendor call.

Your launch plan is probably built around the install date. The date that decides your first 90 days is the one nobody writes down: the day someone can run the device unsupervised, at full protocol, without the rep in the room.

Eight slides on the gap between installed and competent, and the questions that close it before you sign.

Slide 7 is the one to use in the room: ask the vendor to name the week your named provider will be running unsupervised. If nobody commits to that date, you do not have a training plan. You have an install date.

The purchase price is the only number in a device deal that gets negotiated, and it is the least interesting one. It is fixed, it is known, and you sign it once.

Everything that decides whether the machine earns is a number nobody quotes you. Provider hours per treatment. Consumables per treatment. Room time you cannot sell twice. The service line after the warranty. And the retraining bill that arrives the first time the person who ran it leaves.

The purchase price tells you what the machine costs. Those five tell you what an hour in that room has to earn.

Which of the five have you actually priced?

Every RF proposal reads the same.

Sylfirm X, made by Viol: pulsed and continuous radiofrequency at 2 MHz, bipolar non-insulated needles, adjustable 0.3 mm to 4 mm — the manufacturer's published specifications. Two waveforms is also two protocols, and needle tips are a per-treatment consumable.

Price the tips and the training hours before the demo. Save this before your next RF proposal.

The device was right. The order was wrong.

An illustrative teardown — a composite built from patterns we see repeatedly, not a client and not a case study — of the one device mistake that has nothing to do with which device you picked. Capacity to deliver has to exist before the capital to buy, and financing does not resize a calendar.

The test on slide 7 takes about a minute: name the provider, name the hours, and name what those hours currently earn.

Take the Modern Practice Growth Scorecard — link in bio.
