Chronic Ankle Instability & Jump Landing Performance 🏀

Continuing the conversation about how we absorb force during a jump landing, I’m shifting gears to the injury-specific implications.

Ankle sprains are the most common injury in basketball. So common that we don’t give them enough attention, especially when considering the long-term impact.

Up to 40% of lateral ankle sprains develop into Chronic Ankle Instability (CAI). With about ¼ of NBA players sustaining an ankle sprain each season and a 1.41× higher recurrence risk for those with a prior history. This is a problem that demands a closer look.

CAI isn’t just an unstable ankle. It changes the way the entire kinetic chain loads and has implications that extend to the central nervous system.

In this post I break down:

2️⃣ The burden: How common it is in basketball
3️⃣ The buffer: Why the ankle’s role during landing matters so much
4️⃣ The effects: Injury risk and performance cost when that system breaks down
5️⃣ The playbook: What we actually do about it

By maximizing ankle function during the loading phase and minimizing injury risk, we can reduce the performance trade-offs discussed in the last post and keep athletes on the court.

Jump Landing Performance Playbook 📋

My last post discussed the 3 phases of a jump landing and the trade-offs between health and performance. If we want to move the needle, we have to know which phase needs to be addressed and how we are training it.

In this video, I demonstrate various exercises to help improve landing performance based on the goals identified with the athlete:

1️⃣ Pre-Landing Prep
During the anticipation for initial contact, the ankle should approach the ground with some plantar-flexion. The skill of co-contract at the right time requires a great deal of attentional focus to develop.

2️⃣ Loading Phase
High forces are absorbed here during a short duration. We are training the foot and ankle to act as an airbag, using the calf complex to hand off force up the chain.
A combination of developing the local physical qualities of the foot and ankle to handle high forces along with exercises which expose the area to fast loading will be effective in improving the performance here

3️⃣ Attenuation Phase
While most of the work happens here, its success depends on the phases above.
• Performance: High-performers use a “stiffer” strategy with a smaller ROM to get back into the play faster.
• Health: We also need the capacity to “sink” into a deeper position for those chaotic, off-balance landings. 

Finally, part practice is a great tool, but context is king. Training these qualities in isolation is helpful but putting them to the test on-court in a chaotic environment can’t be missed

Jump Landing - Phases & Trade-offs 📈

Health and performance trade-offs are everywhere in sport. Encouraging one action is often at the detriment to something else. The same is true when considering landing performance from a vertical jump. Recognizing the trade-offs allow us to rehab and train specific to the individual players needs and the demands of the game. 

Using the framework from Harry et al. (2024), we can break a landing into 3 phases:

1️⃣ The Loading Phase
In the first 0.05s of contact, forces spike to 7x body mass.
High performers minimize work here by preparing for contact before they hit.
A plantar-flexed landing acts like an airbag, storing elastic energy to decrease some demand on the proximal joints (Toussaint et al., 2024).
However, this position is an open-pack joint position making the ankle more vulnerable to lateral ligament sprains.

2️⃣ The Attenuation Phase
This is where you absorb peak force and find stability.
A shorter, stiff window is faster and more economical for the game.
However, a shorter window increases loading rate on the patellar tendon. 
A deeper ROM is your buffer in chaotic moments, like landing in a high traffic paint.

3️⃣ The Control Phase
Decide on dissipating the stored energy to stand back up or utilize the elastic stores to react to the next action

Spectrum of Force Absorption 🏀📉

In an elite basketball game, a single player can jump 50+ times, but that number doesn’t tell the full story because no two jumps are the same. Everything from the player’s jump strategy to the chaotic demands of the game will influence the force absorption requirements.

Rauch et al. (2020) showed that jump strategies differ between NBA players, as they tend to select one of three internal jump strategies:

1️⃣ Stiff Flexors: High tendon stiffness and short ground contact times. Elite elastic energy storage and rapid turnover.
2️⃣ Hyper Flexors: Utilize deep triple flexion to create extension. They lean on muscular force to get off the floor.
3️⃣ Hip Flexors: Characterized by more displacement at the hip relative to the knee. More commonly seen in larger athletes managing higher mass.

Within-player variability should also be considered:
- A Stiff Flexor may be forced into a deep, long braking phase to time a lob.
- A Hyper Flexor may have only a split second to get off the floor before a defender contests their shot.

Along with jump strategy, Talpey et al. (2021) highlighted that the context of the jump will impact the physical demand on the body. These variations require different speed-strength and reactive strength qualities:
- 1 vs. 2 feet takeoff - Stationary vs. Run-up - Maximal vs. Submaximal effort - Direction of takeoff and landing

While we should feed the beast and play to an athlete’s superpower, we must respect the other end of the spectrum. If a player only has one solution for force absorption, they are vulnerable when the game demands another. For clinicians and coaches, it all starts with assessing where your athlete sits on this spectrum and recognizing that a one size fits all approach isn’t the way to go.

Hip Extension - Power 🚀

🏃🏻 Powerful hip extension is foundational for athletic performance — sprinting, jumping, and changing direction.

✖️ Power is built on the relationship between force and velocity, and by adjusting those variables, we can target different ends of the power spectrum.

💨 Banded drills sit lower on the force side but allow me to express maximal speed — think quick, reactive movement.

🏋🏻 Barbell lifts load up the force side, building strength through range, even if some velocity is sacrificed.

🏀 For sports like basketball, where performance depends on both ends of that spectrum, it’s important to train it all — low-force, high-velocity… high-force, low-velocity… and everything in between.

💪🏽 Hip Extension - Strength 

🌟 A few posts ago, I discussed the importance of hip extension and shared exercise options to improve mobility, motor control, and work capacity. This video highlights the next step in the exercise prescription continuum—strength!

📈 The glute max, adductor magnus, and hamstrings are the primary hip extensors, generating more torque at the hip than any other muscle group. This strength plays a key role in accelerating the body upward (rising from a squat or jumping) and forward (walking uphill or sprinting). While optimizing daily function and athletic performance involves multiple factors, strengthening these muscles enhances force production and overall movement efficiency.

🏋🏻 Building a solid strength foundation with these exercises will set the stage for power-focused movements, which I’ll cover soon!

Who’s ever had an ankle sprain? 🙋🏻‍♂️ 

🫨 Lateral ankle sprains account for nearly 25% of all injuries in the NBA! Crazy thing is that number is probably even higher because so many of them go unreported. Many players simply “walk it off” or “wait and see”. With recurrence rates as high as 75%, it is crucial to take it seriously since they can lead to missed games and performance declines. 

🏀 This provides a glimpse into a mid to late-stage rehab program designed to help a recent client transition back to the court. The exercises are integrated with a gradual return-to-play program to ensure the client is prepared for the demands of the game.

💪🏽 These exercises work on foot and lower leg strength in compound movements, as well as dynamic qualities such as lateral deceleration and building resilience landing at different angles. 

🔑 The selection and programming of exercises should be done at an individual level based on the clients needs
