If you’re thinking about medical school but worried it’s too late, it’s not.

And I didn’t become a physician until 2015.

There is no perfect timeline.

☠️ THALLIUM POISONING — What You Need to Know
🧪 What is it?
An odorless, tasteless heavy metal — historically called “the poisoner’s poison” — used in rodenticides and manufacturing. Still a means of criminal poisoning worldwide.
🔴 The Classic Triad:
GI symptoms (hours): Vomiting, severe abdominal pain — initially looks like any GI illness
Painful neuropathy (days): Burning pain and weakness ascending from the legs, paresthesias, dysesthesias
Hair loss (2–3 weeks): Diffuse alopecia with characteristic darkening/blackening of hair roots

⚠️ Other findings:
Mees lines on nails
Skin pigmentation changes
Delirium, coma, and visual loss in severe cases
Psychiatric symptoms can persist even after recovery

💊 Treatment:
Prussian blue (Radiogardase) — the specific antidote — binds thallium in the gut and blocks reabsorption. Dose: 3g orally three times daily in adults
Hemodialysis/hemoperfusion — especially useful in the first 48 hours
Potassium supplementation
Continue treatment until 24-hour urine thallium is less than 5 mcg/L

🧠 Clinical Pearl:
Think thallium in any patient with rapidly progressive painful peripheral neuropathy + alopecia — especially if the onset was preceded by unexplained GI illness. It’s rare, but missing it can be fatal. taken in Tractor Supply by @heystevemd
