My cardio short case was Tricuspid valve IE - if you suspect tricuspid regurgitation, check for track marks

Expose patient adequately from the beginning. Feel free to cover back some areas when you’re done with your exam on those parts.

Smoothly transitioning the patient to different positions is key to appearing slick. I start from getting the patient to sit normally to check hands and face, then switch to 45° for examination of neck and precordium, then left lateral for MS, sit back up for AR, then listen to the lungs. Lie back down for abdominal exam and lower limbs exam.

Heart sounds auscultation should be in conjunction with palpation of a radial (if strong) or carotid pulse, so to time the murmur whether it happens in diastole or in systole. When listening to the precordium, give the patient short and clear instructions. “Breathe IN. Stop.” And listen for rIght sided (TV/PV) murmurs which will become louder (INspiration - r”I”ght heart). “Breathe out. Stop.” Listen for left sided murmurs as they become louder (EXpiration - lEft heart valves - MV/AV). You don’t need to give any longer instructions than those three words to avoid patient’s confusion.

When you’re feeling for thrills at aortic, pulmonary, tricuspid, mitral area use your finger tips and palm simultaneously if you want to save time. Thrills are very loud murmurs that suggest a severe valvular abnormality. Heave is the thickness of the RV wall from hypertrophy that pushes againstthe chest. When you feel for the parasternal heave, you really have to rest your whole palm and cradle the parastatal area to feel the heave pushing back at you.

Also metallic click is something heard best standing next to the patient rather than on auscultation. It sounds like a ticking clock 🕰️

Special manoeuvres are there to pin down the murmur

Fun facts: I love listening to diastolic AR murmur and mid systolic click for MVP
