🦷 **Today’s case: Upper molar extraction + sinus management + PRF + bone graft — all in one appointment.**

Before touching this tooth, the CBCT scan told me everything I needed to know. 3D imaging isn’t just a “nice to have” — it’s how we evaluate sinus proximity, bone volume, root anatomy, and cortical plate thickness *before* we ever pick up an instrument. In cases like this one, where the root apices are sitting right against the Schneiderian membrane, that information is the difference between a routine extraction and a sinus communication.

Good surgical planning = no perforation today. ✅

Because of how we approached the extraction — careful, controlled, respecting the anatomy — we avoided an oroantral communication. But we weren’t done. With the sinus floor this close, we needed to protect and prepare that site for a future implant.

That’s where **PRF (Platelet-Rich Fibrin)** came in. Using the patient’s own blood, spun in a centrifuge, we created a natural fibrin membrane — concentrated growth factors, no synthetics. This membrane was used in a **sandwich technique**: layered with bone graft material to cushion and support the sinus membrane from below while filling the socket. The result? A biologically optimized environment for new bone formation.

The goal: a fully regenerated, implant-ready site in the months ahead. 🦴

Every step was intentional. Every decision was made *before* we started, not during. That’s what CBCT imaging makes possible. by @luislerzundy
