EPISODE 10 | OBSERVATION BEFORE TOUCHING THE PATIENT 👀

Your physiotherapy assessment starts before you touch the patient.

As a DPT student or physiotherapy student, train yourself to observe first:

👀 Posture: alignment, asymmetry, positioning

🚶 Gait: walking pattern, weight-bearing, limping

⚖️ Symmetry: compare both sides

🛡️ Protective movement: guarding, avoidance, hesitation

😣 Pain behaviour: facial expression, movement speed, discomfort

The purpose of clinical observation isn’t to diagnose immediately.

It helps you create a clinical hypothesis and decide what to assess next.

Think like this:

OBSERVE → HYPOTHESIZE → ASSESS → CONFIRM → REASSESS

Instead of performing every ROM test, muscle strength test, special test, and palpation technique without a plan, let your observation guide your clinical assessment.

This is the foundation of clinical reasoning in physiotherapy.

📌 Save this for your DPT practicals.

Static vs Dynamic Observation 👀

A patient can look completely normal at rest… but the problem may show up the moment they move.

Static observation: What you notice at rest -> posture, alignment, swelling, asymmetry.

Dynamic observation: What changes during movement -> gait, squat, reaching, or functional activities.

The real clinical skill is knowing what to look for and when to look for it. 🧠

Save this for your next DPT practical or clinical assessment.

Knee pain isn’t random. 🦵

Running 🏃‍♂️ Stairs ⬆️⬇️ Squats 🏋️‍♀️

Same triggers → same pattern.

This isn’t just pain…

it’s load ⚖️, tracking 🎯, and control 🧠 repeating.

Stop guessing ❌

Start connecting 🔗

⚠️ Not a diagnosis, just a clinical direction.

Save this, you’ll recognize it next time. 👀

🔷Most DPT students don’t miss the diagnosis because they lack knowledge.

🔷They miss it because they start thinking too late.

🔷Clinical reasoning doesn’t begin during the physical examination, it begins during history taking.

🔷Every symptom, every aggravating factor, every easing factor, and every functional limitation is a clinical clue that helps you narrow your differential diagnosis before you even perform a special test.

🔷The patient’s story is data.
Learn to collect clues, identify patterns, and think like a physiotherapist, not just memorize special tests.

💬 Comment “HISTORY” if you’re committed to improving your history-taking and clinical reasoning skills.
