My name is Professor Martin Sillem. I am a consultant gynaecologist working in both Germany and Dubai.

When you come to see me, the consultation always starts with your perspective.

✅ What is bothering you?
✅ What hurts?
✅ What worries you?
✅ What feels different?

Your experience matters more than assumptions or theories.

From there, we proceed with a careful clinical examination and, in most cases, an ultrasound. Together, these help us reach a preliminary diagnosis. We then review the findings side by side and discuss a clear, sensible plan moving forward.

If you value a structured, evidence-based, and patient-centred approach, you are welcome to see me in clinic.

Most fibroids do not require surgery.

Surgery is more strongly considered when bleeding becomes heavy enough to cause anemia, pain or pressure significantly affects daily life, fibroids affect the bladder or bowel, or fertility or pregnancy plans are affected.

Before any decision is made, evaluation typically includes pelvic ultrasound, MRI when more detailed mapping is needed, and bloodwork if bleeding is heavy.

Treatment is not one-size-fits-all. Medication, radiofrequency ablation, myomectomy, or hysterectomy depends on age, fertility goals, and the size, number, and location of the fibroids.

Before agreeing to surgery, the important question is: what will be different afterward?

If fibroids are changing or symptoms are worsening, an expert consultation can help clarify whether surgery is the right conversation to have.

When is the right time to freeze your eggs?

For some women, the answer becomes especially important before cancer treatment. For others, it comes down to planning for a future pregnancy.

This reel explains why timing matters. taken in Dubai, United Arab Emirates by @martinsillem

At 99 years of age, she recovered from surgery remarkably quickly, baked a cake, and brought it to the hospital herself.

But that wasn’t even the most surprising part of meeting her.

When I asked her how she had managed to stay so fit at 99, her answer caught me completely off guard.

Pelvic floor dysfunction does not cause endometriosis.

But ongoing pelvic pain from endometriosis can cause the pelvic floor muscles to guard and tighten over time. That guarding can become a second source of pain — and may persist even after endometriosis is treated.

Expert assessment is important to identify whether pelvic floor dysfunction is also contributing to persistent pain and to guide the appropriate treatment approach. taken in Dubai Healthcare City by @martinsillem

Perimenopause isn’t a straight line—and that’s what makes it so confusing.

Hormones can fluctuate significantly during this stage, and ovulation may become less regular before ovarian activity gradually declines.

That’s why your periods, symptoms, and even how you feel can seem different from one day to the next.

And when it comes to assessing perimenopause, there isn’t always a single hormone test that tells the whole story. Your symptoms, menstrual pattern, age, and overall health all matter.

If something has changed in your cycle or how you feel, don’t dismiss it. Come for a consultation. taken in Dubai, United Arab Emirates by @martinsillem

My experience with Professor Sillem was exceptional. He combined expertise, decisiveness, and patient empathy, taking the time to explain every step of my care clearly in English. Referred by my gynaecologist, who spoke highly of him, he quickly identified the root cause of my condition, provided a thorough assessment, and agreed a treatment plan during our first consultation.

Within 14 days I underwent my complex surgery. I was particularly impressed by the care and skill shown during the procedure, which was performed through existing wounds to avoid additional scarring despite the added complexity.

Follow-up care was equally professional, with a comprehensive explanation of the procedure and a clear recovery plan covering medication, consultations, and next steps.

I am deeply grateful to Professor Sillem and his team and give them my highest recommendation for outstanding patient care, complex surgical expertise, and excellent follow-up support.

Getting a second opinion on an endometriosis diagnosis is a normal and accepted part of managing a complex chronic condition.

It may be worth considering if your symptoms persist despite treatment, your scan came back “normal” but you are still experiencing symptoms, you were told nothing more can be done, or you feel rushed, dismissed, or unheard.

A second opinion can offer a fresh clinical perspective, including a review of your previous imaging, notes, and treatment history. In some cases, it may also identify a different treatment or surgical approach that has not previously been discussed with you.

Bring any prior scans, surgical or procedure reports, and a timeline of your symptoms. This can help the second specialist understand your case without starting from zero.
If something still does not feel resolved, seeking another perspective is a reasonable step.

Chronic pelvic pain isn’t always coming from where you think.

In my practice, I often see patients who have been dealing with pain for a long time without a clear explanation. Sometimes, the answer lies in connections within the body that are easy to overlook.

Understanding these connections can be an important step toward finding the right approach to treatment.

If you experience persistent pelvic pain, it’s worth looking beyond the obvious. by @martinsillem
