The human lower limb skeleton consists of **30 distinct bones** designed to support body weight, provide stability during standing and walking, and facilitate locomotion. Anatomically, the lower limb is divided into four main regions: the pelvic girdle (hip bone), the thigh, the leg, and the foot.
### 1. Pelvic Girdle (Hip Bone / Coxal Bone)
Each lower limb attaches to the axial skeleton via the **hip bone** (os coxae). During childhood, the hip bone consists of three separate bones that eventually fuse during puberty at the **acetabulum** (the cup-shaped socket for the hip joint).
 * **Ilium:** The superior, fan-shaped portion. Features the prominent **iliac crest**, anterior superior iliac spine (ASIS), and posterior superior iliac spine (PSIS), serving as critical attachments for trunk and hip muscles.
 * **Ischium:** The posteroinferior portion forming the "sit bone." Includes the heavy **ischial tuberosity** (which bears weight when sitting) and the **ischial spine**.
 * **Pubis:** The anteroinferior portion. It features superior and inferior rami that meet at the midline to form the **pubic symphysis** (a fibrocartilaginous joint) and contributes to the obturator foramen.
### 2. Thigh Bone (Femur)
The **femur** is the longest, heaviest, and strongest bone in the human body.
 * **Proximal End:** Features a spherical **head** that articulates with the acetabulum, a constricted **neck** (a common site of osteoporotic fractures in older adults), and two large roughened prominences: the **greater trochanter** (lateral) and the **lesser trochanter** (medial), which serve as powerful lever arms for hip rotators and flexors.
 * **Shaft:** Slightly bowed anteriorly to absorb compressive forces, with a posterior longitudinal ridge called the **linea aspera** for extensive muscle attachment.

Definition: Spondylolysis is a defect or stress fracture in the pars interarticularis of a vertebra, most commonly at L5.

### Causes

* Repetitive stress and microtrauma.

* Repeated lumbar extension and rotation.

* Sports such as gymnastics, football, and weightlifting.

### Clinical Features

* Low back pain, especially during lumbar extension.

* Pain increases with sports or physical activity.

* Local tenderness over the affected vertebra.

* Hamstring tightness may be present.

### Diagnosis / Investigations

* X-ray: Oblique view may show the Scottie dog sign with a defect or collar.

* CT scan: Best for detailed assessment of the bony defect.

* MRI: Helps detect an early stress reaction and assess soft tissues.

### Management

* Conservative treatment: Rest from aggravating activities and activity modification.

* Physiotherapy: Core stabilization, gradual strengthening, flexibility exercises, and avoiding painful lumbar hyperextension.

* Bracing: May be considered in selected cases.

* Surgery: Rarely required if symptoms persist despite appropriate treatment.

### Important Exam Point

Spondylolysis = defect in the pars interarticularis.

If the affected vertebra slips forward over the vertebra below it, the condition is called spondylolisthesis. by @medsurgeryai
